| Objective:Because of the Angle class Ⅱ malocclusion complicated causes,clinical manifestations varied,has been a hot and difficult field of orthodontics.Many clinical study showed that a part of class Ⅱ malocclusion deformity patients have temporomandibular joint problems in different degree,which requires dentists not only pay attention to the teeth arrangement,should think about how to adjust the mandibular position for temporomandibular joint health,functional coordination to pursue the stomatognathic system in improving the occlusal relationship at the same time.In this study,class Ⅰ and class Ⅱ malocclusion patients temporomandibular joint skeletal structure by cone-beam CT.3D reconstruction of the structure by Mimics,a representative project measurement and analysis,statistical comparison,in order to find the relationship between fault types and skeletal structure of temporomandibular joint function,so as to guide the clinical design and orthodontic treatment.Method:Select 60 patients who with the Angle class Ⅰ and Ⅱ malocclusion as the research object.According to the malocclusion type,divided into 3 groups,including 20 cases of class Ⅰ patients,include 10 men and women;class Ⅱ division 1 group of 20 patients,include 10 men and women;class Ⅱ division 2 group of 20 patients,include 10 men and women,the average age of 19.8.All the patients signed the informed consent,captured the cone beam CT,reconstructed the 3D morphology with mimics,and selected 13 representative items for measurement,such as:anterior joint space,superior joint space,posterior joint space,depth of articular,height of condylar,anterior oblique angle of condylar process,posterior oblique angle of condylar process,anteroposterior diameter,angles of posterior joint node bevel,internal angle of condylar process,external angle of condylar process,length of the long axis of condyle,angle of the long axis of the condyle.Finally,the data classification,using statistical software SPSS 17.0 analyze the temporomandibular joint morphology comparison of 3 types of malformation.Results:Class Ⅰ,class Ⅱ division 1 group,class Ⅱ division 2 groups of left and right side data comparison,data were not statistically significant(p>0.05).Class Ⅰ,class Ⅱ division 1 group,class II division 2 groups of male and female data comparison,data were not statistically significant(p>0.05).Class Ⅰ group,class Ⅱ division 1 group,class Ⅱ division 2 group on anterior joint space,superior joint space,posterior joint space,the differences were statistically significant(p<0.05).Angle class Ⅰ group and class Ⅱ group,the differences between anterior oblique angle of condylar process and angles of posterior joint node bevel were statistically significant(p<0.05).Conclusion:1.Angle class Ⅰ,class Ⅱ division 1,class Ⅱ division 2 malocclusion,structure of the left and right joints of the patient is basically symmetrical.Compared to Angle class Ⅰ and class 2 malocclusion,there is no sexual difference in the temporomandibular joint.2.The locations of condylar joints in the Ⅰ class,the Ⅱ classification 1 and the class 2 classification malocclusion were different in the joint fossa.3.From the class Ⅰ and class Ⅱ patients anterior oblique angle of condylar process and angles of posterior joint node bevel of the variation,mandibular position affect the temporomandibular joint morphology to a certain extent. |
