Aim : Explore the value of parameters obtained by noninvasive hemodynamic monitoring using thoracic electrical bioimpedance in the assessment of cardiac function in children with persistent pulmonary hypertension.Methods: Based on whether they had persistent pulmonary hypertension,sixty children(full-term infants,gestational age >37 weeks,weight >2.5 kg)enrolled between January 2017 and December 2018 were selected and divided into PPHN groups(30 children)and control group(30 children).Three groups of neonates were tested with color Doppler ultrasound and noninvasive hemodynamic monitoring using thoracic electrical bioimpedance.The SV,CO,STR,ICON,TFC,FTC of neonates were obtained through noninvasive hemodynamic monitoring.The RVDD,LVEDD,LADD,RVLD,RVWD,MPAD,EF of neonates were obtained using color Doppler ultrasound.All parameters above were analyzed for differences and correlations.Results:(1)Cardiac color ultrasound data: The RVDD,MPAD,RVWD,RVLD of PPHN group are all significantly higher than those of the control group,P<0.05,differences were statistically significant.EF of two groups were not significantly different.(2)Cardiac color ultrasound data: The RVDD,MPAD,RVWD,RVLD of PPHN group were all significantly higher than those of the control group,P<0.05.EF of two groups were not significantly different.(3)Correlation analysis between results of thoracic electrical bioimpedance and echocardiographic in PPHN group: SV,CO was negatively correlated with MPAD,RVWDand RVLD.SV,CO was positively correlated with LVDD and LAD.STR was positively correlated with MPAD,RVWD,RVLD;FTC was negatively correlated with LVDD and LAD.ICON and TFC were not correlated with the other datas.Conclusions: In children of PPHN group,there is usually no significant decrease in EF before cardiac dysfunction occurs.Heart color Doppler ultrasound detection can detect changes in the diameter of the room.And thoracic electrical bioimpedance can effectively detect the hemodynamic changes in children with PPHN.While heart color Doppler ultrasound are unable to detect changes in EF,thoracic electrical bioimpedance can detect changes in SV,CO,STR and TFC,and abnormal changes of parameters more sensitively.Hemodynamic monitoring using horacic electrical bioimpedance can evaluate the cardiac function status of children.The thoracic electrical bioimpedance is well correlated with the echocardiographic parameters,and the two can effectively complement each other,providing more hemodynamic parameters for the assessment of cardiac function of children with PPHN.
