| Objectives:to investigate the treatment of middle aged ruptured intracranial aneurysm,including microsurgical clipping and endovascular embolization,and to compare the clinical effects of the two treatments.Methods:clinical data of middle-aged patients with ruptured intracranial aneurysms admitted to the neurosurgery department of Handan first hospital from September 2016 to July 2018 were retrospectively analyzed.Inclusion criteria and exclusion criteria were set,and clinical data of 90 middle-aged patients with ruptured intracranial aneurysms were collected.According to different treatment methods,45 patients in the study group and 45 patients in the control group were divided into two groups.The middle-aged patients in the study group were treated with microsurgical clipping,while the middle-aged patients in the control group were treated with endovascular embolization.Finally,the postoperative treatment effect,complications and daily life ability of the two groups were compared.Results:the common complications of the two groups,including cerebral infarction,hydrocephalus,intracranial infection,aneurysm rupture,and the incidence of complications were compared.The incidence of complications in the study group was 28.9%,higher than that in the control group 8.9%,and the difference was statistically significant(P<0.05).One month after surgery,the GOS prognostic score scale was used to evaluate the treatment effect.The good rate in the study group was55.6%,which was lower than that in the control group 75.6%,and the difference was statistically significant(P<0.05).The improved Rankin scale was used to evaluate the daily living ability 3 months after surgery.The proportion of patients with independent living ability in the study group was 77.8%,lower than that in the control group 95.6%,and the difference was statistically significant(P<0.05).Conclusions:endovascular embolization and microsurgical clipping are effective treatment for middle-aged patients with ruptured intracranial aneurysms.When both treatment methods are suitable for middle-aged ruptured intracranial aneurysm,endovascular embolization has advantages over microsurgical clipping treatment in terms of treatment effect,complications and daily living ability,which has certain clinical reference value. |
