Clinical Study On The Relationship Between TCM Syndromes And Diagnosis Of Rectal Cancer Based On FH Test

Objective:Early screening of suspected rectal cancer cases was carried out by using modern tests based on traditional Chinese medicine literature.Analyzing the results of these patients,we can conclude the relevance between TCM syndrome and infection of rectal cancer,also the relevance between TCM syndrome and the clinical diagnosis of rectal cancer.These cases can provide scientific basis for the accuracy of TCM diagnosis of rectal cancer,and also provide the best method for early screening.Method:This paper mainly adopts prospective experimental study.TCM syndromes of 117 eligible subjects in our hospital were collected.FH test in rectal epithelial cells,fecal occult blood test and combined detection were given respectively.The data of clinical indicators of subjects and laboratory indicators of three tests were collected and then we use SPSS22.0 to process data.Results:?1?In the comparison of TCM syndromes of rectal cancer cases detected by FH detection,FOBT detection and FH+FOBT combined detection,by Rank Sum Test designed by random block,M=83.67>M_0.05,?3,3?=18,p<0.05,the difference was statistically significant.That is to say,in the three groups of cases detected as rectal cancer,the distribution of TCM syndromes is mainly intestinal dampness-heat syndrome,followed by Qi-blood deficiency syndrome and Qi stagnation-toxin-stasis syndrome.?2?The Chi-square test is used for the diagnosis of TCM syndromes and rectal cancer,?²=2.572,p=0.632>0.05,so the difference has no statistical significance.The three main TCM syndromes mentioned above have no obvious significance in the diagnosis of rectal cancer.?3?The sensitivity of FH,FOBT and FH+FOBT were 80%,70%and 60%respectively.Chi-square test showed that?²=0.966,p=0.617>0.05.There was no significant difference in sensitivity of FH,FOBT and FH+FOBT in detecting rectal cancer.?4?In the detection of rectal precancerous lesions,the Chi-square test showed that?2=3.452,p=0.178>0.05,with no significant difference.?5?The specificity coefficients of FH group,FOBT group and FH+FOBT group are respectively:84.4%,63.6%and 92.8%?FH+FOBT group>FH group>FOBT group?.In Chi-square test,?²=31.424,p=0<0.05,so there is significant difference in the detection of specificity.?6?The Youden indices of the three groups are respectively:0.644,0.336 and 0.528.The PV+indices are respectively:32%,14.9%and 42.9%.The PV-indices are respectively:97.9%,95.9%and 96.3%.The total consistent rates are respectively:84%,64.2%and 90.1%.Conclusions:?1?The distribution of TCM syndromes is related to the clinical and pathological diagnosis of rectal cancer.It is in accordance with the clinical pathogenesis of rectal cancer,which is characterized by"marked solid"in the early stage and"benxu"in the middle and late stage,and intestinal dampness-heat syndrome in the early stage;The distribution of TCM syndromes in rectal cancer and non-colorectal cancer patients is irregular,and there is no specific TCM syndromes for clinical diagnosis of rectal cancer.?2?The FH test is highly specific,simple and fast in the early screening for rectal cancer,which means it is worth promoting.But the sensitivity of the FH teat needs to be improved.In addition,we need multicenter,large sample cross-sectional data for further analysis.?3?The relationship between FH substance and TCM syndromes of rectal cancer:At present,there is no objective standard for the diagnosis of TCM syndromes of rectal cancer.FH substance can be used as an objective auxiliary index for the diagnosis of intestinal damp-heat syndrome of rectal cancer.However,because the number of confirmed cases of rectal cancer in this experiment is too small and there is no basis for conclusion,a retrospective study can be added on the basis of the original study to further verify the high performance of FH in the diagnosis of intestinal damp-heat syndrome,that is,most of the TCM syndromes of rectal cancer detected by FH are intestinal damp-heat syndrome.
