Background:According to the current results,too much glucocorticoid can lead to steroid-induced femoral head necrosis(SANFH).The median age of the disease is about 39 years old,and the general demand for exercise is high.In addition,hormones are often required to be taken for a lifetime,and the disease often progresses progressively.Once the disease progresses to the collapse stage,only artificial joint replacement is an accurate treatment.At present,in the surgical treatment of hip preservation,the commonly used method is autologous bone transplantation combined with core decompression,and the effect is the most significant.However,due to the limitations of autologous bone donor sites,allogeneic bone,as a clinical alternative material,will also produce a lot of risks.Therefore,currently,there is still a lack of appropriate bone replacement materials.With the continuous development of tissue engineering scaffolds,many solutions are provided for the above problems.Objective:To explore the mechanism of Yougui-Decoction in the treatment of steroid-induced femoral head necrosis,and to provide theoretl basis for the clinl appltion of the prescription.Methods:12 New Zealand white rabbits were randomly divided into 2 groups:Yougui-Decoction group,glucocorticoid control group,each group has 6 rabbits.Another 6 rabbits in the same batch were randomly selected as blank control group.Early femoral head necrosis model was established by using high dose horse serum combined with dexamethasone.After 6 weeks of intervention,three groups of rabbit basic metabolic rate variables were measured,MicroCT was performed to compare the density of bone trabeculae,and the cells in the femoral head were analyzed with HE staining,MASSON staining,TRAP staining and VEGF immunohistocheml analysis.Results:The basal metabolic rate variables and bone trabecula density in the Yougui-Decoction group were signifntly higher than those in the glucocorticoid control group(P=0.003,P=0.021).MicroCT showed that the bone mass of the Yougui-Decoction group was signifntly higher than that of the glucocorticoid control group(P=0.004,P=0.000,P=0.001,respectively).The results of MASSON staining showed that the osteogenic activity in femoral head of both blank group and the Yougui-Decoction group were superior to the glucocorticoid control group,the difference was statistlly signif?nt(P=0.011,P=0.017,respectively).TRAP staining indted the lowest level of the osteoclast active was showed in blank group but no statistl difference with the Yougui-Decoction group(P=0.077),both signifntly lower than the glucocorticoid control group(P=0.000,P=0.009,respectively).Immunohistochemistry of VEGF showed that the angiogenesis ability of the Yougui-Decoction group was better than that of the two control groups,and the difference was statistlly signifnt(P=0.031,P=0.000,respectively).Conclusion:Yougui-Decoction can signifntly improve the basal metabolism and bone density of the model of steroid-induced femoral head necrosis,promote osteogenesis and inhibit osteoclast activity,signifntly improve the ability of intracranial angiogenesis,reflect the effect of "tonifying kidney and heart,strengthening bones and arteries",and reflect the scientific basis of clinYougui-Decoction 1 use.
