Objective:The bone thickness of the alveolar bone in anterior teeth is the thinnest in patients with skeletal class Ⅲ high angle and is the focal point for clinical orthodontists.It has yet to be established how the tongue’s size and position within the oral cavity affects the position of the lower anterior teeth and the thickness of the alveolar bone of the lower anterior teeth.In this study,measurements were taken in order to evaluate the position of the tongue and the alveolar bone of the mandibular incisors through the application of CBCT.This was done in order to understand their correlation,and to provide reference for the evaluation of the alveolar bone morphology of the lower anterior teeth in adults with skeletal class III malocclusion with different lingual postures.Selection methods: from 2016 to 2019,at the orthodontic department of shenyang,division of 18 to 40,39 patients with osseous Ⅲ class high wrong jaw Angle,including13 cases of male,female 25 cases,selected periodontal healthy patients,and no obvious reverse front teeth,crowded degree < 4 mm,teeth without defect.Since the tongue tissue cannot be clearly developed,barium sulfate should be applied to the center of the tongue surface before the shooting of CBCT to achieve better development effect of the tongue tissue.The CBCT data were measured by using INVIVO5.0 image processing software to build 3D images of jawbone and teeth.Selection on the left side of the incisor in measure,on the sagittal images along the mandibular incisor in teeth axis,measuring the labial side under the alveolar ridge top 2 mm(T1),central root(T2)androot(T3)buccal tongue neck at its widest point buccal alveolar bone thickness(T1B1T2B2 T3B2)and lingual alveolar bone thickness(T1L1 T2L2 T3L3),labial side attached to height of the alveolar bone(HB)and lingual alveolar bone height(HL).The distance(D1,D2,D3,D4,D5,D6)from the dorsum of the tongue to the hard palate and the inclination of the lower incisor(IMPA)were measured and converted using a geometric drawing board on the cephalometric lateral film.The measured results were input into Microsoft Excel for sorting,and the sorted data were imported into SPSS 22.0software.pearson correlation analysis was conducted on the distance between the back of the tongue and the hard palate at 6 points of the tongue body,as well as the thickness,height and inclination of the alveolar bone of the lower incisor.The test standard was P< 0.05,which was statistically significant.Results: 1.The six sites pose in a tongue when back to the distance of the hard palate and tongue three levels incised tooth side of your lips alveolar bone thickness correlation analysis,the results showed that under the labial alveolar ridge top level 2mm T1,back to the distance of the hard palate D3,D4 and tongue lip side under the front teeth alveolar bone thickness T1B1 showed a negative correlation(r = 0.413,0.433,P < 0.05),tongue back to the distance of the hard palate D3,D4,D5,and under the front teeth lingual alveolar bone thickness T1L1 showed a negative correlation(r =0.171,0.048,0.069,P < 0.05).At T2,the distance from the back of the tongue to the hard palate was negatively correlated with T2B2(-0.203,-0.213,-0.451,P<0.05)and T2L2(r=-0.190,-0.183,-0.227,P<0.05).At the root apical level T3,the distance D1,D3,D4,D5 from the back of the tongue to the hard palate was negatively correlated with the alveolar bone thickness T3B3(r=-0.559,-0.143,-0.135,-0.213,P<0.05),and the distance D1,D3,D4 from the back of the tongue to the hard palate was negatively correlated with the alveolar bone thickness T3L3(r=-0.203,-0.268,-0.340,P<0.05).2.Correlation analysis was conducted between the distance from the back of the tongue to the hard palate and the attached thickness of the alveolar bone on the lip and tongue side of the lower incisor at six points in the tongue posture,and the results showed that the distance between the back of the tongue and the hard palate,D4,D5,D6,was positively correlated with the attached height of the alveolar bone on the lip side of the lower incisor,HB(r=0.911,0.929,0.934,P<0.05).The distance D5 between the dorsum of the tongue and the hard palate was positively correlated with the attachment height of the lingual alveolar bone of the lower incisor(r=0.897,P<0.05).3.Pearson correlation analysis was performed on the distance from the back of the tongue to the hard palate and the inclination of the lower incisor at the six positions of the tongue posture.The results showed that the distance D4 and D5 from the back of the tongue to the hard palate was positively correlated with the inclination of the lower incisor(r=0.954,0.958,P<0.05).Conclusion: 1.Adult patients with osseous Ⅲ class high Angle,when the distance between the tongue and palate back top is increased,the tongue position is relatively low,the corresponding side of front teeth under your breath alveolar bone thickness thinning.2.Adult patients with osseous Ⅲ class high Angle,when the distance between the tongue and palate back roof increases,the tongue position is relatively low,the corresponding regions before the bone attached height,height of alveolar bone.3.The adult patients with osseous Ⅲ class high Angle,when the distance between the tongue and palate back roof increases,the tongue position is relatively low,the corresponding labial inclination increases under the front teeth. |
