Objective1.To explore the effect of stratified follow-up on self-management ability of patients with type 2 diabetes mellitus.2.To explore the effect of stratified follow-up on glucose metabolism in type 2diabetic patients.Methods This study was a randomized controlled trial.A convenience sampling method was used to select 80 patients with type 2 diabetes mellitus who received treatment in the Endocrinology Department of a third Class A Hospital in Dalian from October 2017 to June 2018.To verify the effect of stratified follow-up on self-management ability and glucose metabolism in patients with type 2 diabetes mellitus.The patients were randomly divided into control group(n = 40)and experimental group(n = 40).The control group was given routine follow-up intervention,and the experimental group was treated with stratified follow-up intervention based on Triangle hierarchical management model.The self-management ability,fasting plasma glucose,2h postprandial plasma glucose and glycosylated hemoglobin of the two groups were evaluated before and after intervention.Results 1.The baseline scores of self-management ability in the control group and the experimental group were(38.42±11.47)and(37.76±11.57),there was no significant difference(P > 0.05).After 6 months' stratified follow-up intervention,the baseline scores of the control group and the experimental group were(38.42±11.47)and(37.76±11.57),respectively.The scores of self-management ability of the two groups were improved,but the improvement of the experimental group was significantly higher than that of the control group,and played a significant role in different risk levels.2.There was no significant difference in the baseline data of fasting plasma glucose,2h postprandial plasma glucose and glycosylated hemoglobin(P > 0.05).Compared with the control group,the fasting plasma glucose,2h postprandial plasma glucose and glycosylated hemoglobin of the experimental group decreased significantly after 6 months' stratified follow-up intervention,but the intervention effect was mainly reflected in the middle and high risk patients.Over time,fasting blood glucose and postprandial 2-hour blood glucose increased in both groups 6 months after discharge,but were still well below baseline data.Conclusion Stratified follow-up can improve the self-management ability of patients,and may have some effects on fasting plasma glucose,2h postprandial plasma glucose and glycosylated hemoglobin in patients with middle and high risk layers.This provides a certain reference for the follow-up management of diabetes mellitus. |
