Objective: To explore the value of spectral CT imaging in evaluating the early efficacy of chemotherapy for advanced Lung Cancer.Methods: To collect 42 cases of lung cancer patients with complete clinical data in our hospital,who has been confirmed by pathology.Before and after after 2 cycles of chemotherapy respectively complete enhanced CT scan of energy spectrum,and then measure tumor entity component in arterial and venous phase,which include iodine(water)concentration,water(iodine)concentration,70 keV CT values and spectrum curve slope,finally calculate difference value between the before and after chemotherapy.Evaluating the chemotherapy response by RECIST1.1 standard that contain complete response(CR),partial response(PR),stable disease(SD)and progressive disease(PD).Evaluating SD cases again after second cycle chemoltherapy by observingthe nextefficacyand analyzing the change of iodine content.Results: The difference of iodine concentration,70 keV CT value and spectrum curve slope before and after chemotherapy in effective group was statistically significant(P<0.05),There was nonsignificant difference in water concentration(P>0.05).The difference of iodine concentration,70 keV CT value and of spectrum curve slope in venous phase before and after chemotherapy in invalid group was statistically significant(P<0.05),.There was nonsignificant difference in water concentration(P>0.05).Comparison of iodine concentration in arterial phase before chemotherapy between effective group and ineffective group,there was nonsignificant difference(P>0.05).Comparison of iodine concentration and iodine difference in arterial phase after chemotherap,there are statistically significant(P<0.05).There was no significant difference in iodine concentration in venous phase before chemotherapy betweeneffective group and ineffective group(P>0.05),but the iodine concentration and iodine difference in venous phase after chemotherapy have significant difference(P< 0.05).There was significant correlation between difference of iodine concentration in arterial phase and venous phase and RECIST1.1 standard(r=0.594,P<001 、 r=0.718,P<001).There are 15 SD cases after 2 cycles chemotherapy,when reviewing next curative efficacy,there are 10 SD cases,3 PR cases and 2 PD case.Conclusion:Gemstone Spectral Imaging has certain clinical value to evaluate the early curative effect of advanced lung cancer chemotherapy. |
