Clinical Characteristics Of Patients With Chronic Viral Hepatitis B Complicated With Hepatic Hydatid

Objective:To study the co-infection of hepatic hydatid and chronic hepatitis B and its clinical characteristics.Methods:Hepatitis B surface antigen(HBsAg)carrying rate in3493 patients with hepatic hydatid disease was retrospectively investigated.The clinical characteristics of patients with coinfection of hepatic hydatid and chronic hepatitis B were analyzed.To evaluate the effect of chronic viral hepatitis B on the surgical treatment of hepatic hydatid disease.Results:The co-infection rate of liver echinococcosis and chronic hepatitis B was 7.76%(271 / 3493),and the carrier rate of hepatitis B surface antigen in patients with cysticercosis was 7.02%(216 / 3079).Hepatitis B surface antigen carrying rate was 13.29%(55 / 414)in patients with alveolar hepatic hydatid disease.Most of the909 patients were male and Han nationality,and the co-infection was also found in young Han patients.Most of the co-infected patients had no specific symptoms and signs,mainly right upper abdominal pain.HBeAg-negative chronic hepatitis B accounted for 81.17%(194)./ 239),HBV DNA detection rate was very low(35 / 271).The liver damage was more severe in patients with coinfection of hydatid liver and chronic hepatitis B than that in patients with non-co-infection.There was a significant difference in TBIL?ALT?AST?BA between the two groups(P < 0.05).Compared with the patients with cystic hydatid liver,the liver damage of alveolar hydatid and chronic hepatitis B co-infection was more severe.There was significant difference in A,alt and AST?AKP?GGT?BA between the two groups(P < 0.05).There was a significant difference between the two groups after operation(P < 0.05),and there was a significant difference between the two groups(P <0.05).Conclusion:The carrier rate of hepatitis B surface antigen in patients with hydatid liver was similar to that of the general population in China,but the carrier rate of hepatitis B surface antigen in patients with alveolar hydatid liver was significantly higher than that in the general population.Co-infection of patients with severe liver damage,especially inpatients with alveolar liver hydatid,may increase the risk of surgery,worthy of further study.
