Objective: To investigate the volumetric parameters of the cerebellopontine angle(CPA)cistern as a pathogenic factor of primary trigeminal neuralgia(PTN)and the prognostic factor after of microvascular decompression(MVD).Methods: the clinical data and area deviation of 54 patients with PTN treated by MVD were retrospectively analyzed,and the area deviation was defined as the cross-sectional area parameter of CPA.Results:Comparison of CPA cross sectional area of PTN patients ipsilateral and contralateral,The cross sectional area of CPA in the affected side of PTN patients was smaller than that in the healthy side,and there was statistical significance(P=0.002),but there was no statistical significance between the two sides of the control group(P > 0.05);The area deviation of CPA pool in PTN patients was smaller than that in control group,there was statistically significant(P < 0.001).The area deviation of recurrent group was smaller than that of non-recurrence group,and there was statistical significance(P < 0.022).Patients with CPA were more likely to recur in the left side than those on the right side,with statistical significance(P = 0.018).Conclusions: The smaller area of CPA and the smaller deviation of area of CPA are the pathogenic factors of PTN.The patients with low deviation of CPA area were treated with MVD,which indicated that the effect of MVD treatment was worse than that of other patients.CPA is more likely to recur on the left side than on the right.
