Objective:Meniere's disease is a common disease in the ear diseases and by vertigo,sensorineural hearing loss,tinnitus and(or)aural fullness.The etiology and pathogenesis of Meniere's disease are still unclear.At present,there are many theories,such as endolymphatic longitudinal flow theory,endolymphatic absorption disorder,vascular factors,viral infection,autoimmunity,genetic genes and so on.Clinical treatment is mainly to alleviate the symptoms,and it is still difficult to cure completely.It has a great negative impact on the patients' psychological condition and quality of life.Therefore,it is of practical significance to study Meniere's disease in depth.In recent years,the relationship between Meniere's disease and the immune system has been paid attention by scholars,and it is one of the leading researches in the field of Meniere's disease.Experts and scholars speculate that Meniere's disease may be an immune-mediated or even an autoimmune disease.In this study,we want to investigate the difference of immune microenvironment between Meniere's disease patients and healthy volunteers,to study the role of Treg subsets and their related cytokines in Meniere's disease and evaluate the clinical correlation and significance,to further explore the pathogenesis of Meniere's disease and provide data reference for follow-up immunotherapy.Methods:1.We collected 55 patients with Meniere's disease in Shandong Otolaryngology Hospital affiliated to Shandong University from June 2018 to December 2018,and selected 28 healthy volunteers whose age and sex matched with the population of patients with Meniere's disease.2.Detect the proportion of Treg cells to CD4+T cells in peripheral blood of patients with Meniere's disease and healthy volunteers by flow cytometry.3.Detect the levels of TGF-? cytokines in plasma of patients with Meniere's disease and healthy volunteers by enzyme-linked immunosorbent assay.4.Detect the relative levels of Foxp3 and TGF-? mRNA in peripheral blood of patients with Meniere's disease and healthy volunteers by real-time quantitative PCR.Results:1.The proportion of Treg cells to CD4+T cells in peripheral blood of healthy control group and Meniere's disease group.The proportion of Treg cells in Meniere's disease group was significantly lower than that in healthy control group(P<0.05).2.The TGF-? levels in healthy control group and Meniere's disease group.The level of TGF-? in Meniere's disease group was significantly higher than that in healthy control group(P<0.05).3.The relative expression levels of TGF-? and Foxp3 mRNA in peripheral blood mononuclear cells of healthy control group and Meniere's disease group.The relative expression level of TGF-? mRNA in peripheral blood of patients with Meniere's disease was significantly higher than that in healthy controls(P<0.05).It was consistent with the trend of the level of TGF-? in plasma.The relative expression level of Foxp3 mRNA in peripheral blood of patients with Meniere's disease was significantly lower than that in healthy control group(P<0.05).It was consistent with the trend of the Treg cells in peripheral blood.4.Correlation between the Treg cells in peripheral blood and the level of TGF-? in plasma with clinical factors in patients with Meniere's disease.The main results were as follows:(1)Gender:the proportion of Treg cells in peripheral blood of female patients with Meniere's disease was higher than that of male patients with Meniere's disease,but there was no significant difference between the two groups(P>0.05).(2)Age:compared with patients under the age of 50 with Meniere's disease,the proportion of Treg cells in peripheral blood of patients?50 years old was slightly higher and the level of TGF-? in plasma was slightly lower,but there were no significant difference between the two groups(P>0.05).(3)Clinical classification:the proportion of Treg cells in peripheral blood of patients with type ? Meniere's disease was lower than that in patients with type ? and ?Meniere's disease,The plasma level of TGF-? in type IV patients was lower than that in type ? patients,but there were no significant difference(P>0.05).(4)Unilateral and bilateral:compared with patients with unilateral Meniere's disease,the proportion of Treg cells in peripheral blood and the level of TGF-? in plasma were higher in bilateral patients,but there were no significant difference(P>0.05).(5)Hearing:there was no correlation between the Treg cells in peripheral blood and hearing in patients with Meniere's disease(P>0.05).Conclusion:We found that in patients with Meniere disease,the proportion of Treg cells and the relative expression levels of Foxp3 mRNA decreased;at the same time,the level of TGF-? in plasma and the relative expression levels of TGF-? mRNA increased,suggesting that Treg cells and TGF-? may be involved in the pathogenesis of Meniere's disease and are associated with decreased immunosuppressive function in patients.
