analysis of endpoint events in patients with myocardial infarction during hospitalizationObjectivesObserve the situation of traditional Chinese medicine treatment and end point events in different subgroups of myocardial infarction patients during hospitalization,explore the correlation about them,and provide evidence-based medical evidence for clinical curative effect evaluation of traditional Chinese medicine.MethodsThrough the hospital's medical record information management system,the patients who were hospitalized with the first myocardial infarction at 8 Chinese TCM hospitals were selected and the questionnaire was completed.The data were entered into the Epidata database of a cohort of patients hospitalized for myocardial infarction in a Chinesemedicine hospital established by the Myocardial infarction study group.SPSS 22.0 and Logistic regression analysis were used for different subgroups of myocardial infarction(including myocardial infarction with hypertension,diabetes,and dyslipidemia).The correlation analysis of the patients' TCM intervention and the occurrence of the end event.Results1.Investigate 1,596 inpatients,with an average length of stay of 15 days;among them,60.40% are associated with different degrees of hypertension,46.62% are associated with diabetes,and patients with different types of dyslipidemia account for 36.59%.TCM syndromes are the most common with qi deficiency and blood stasis syndromes,a total of 878 cases,accounting for 55.01%.2.Multivariate Logistic regression analysis showed that The related factors associated with Cardiogenic death during hospitalization in patients with myocardial infarction and hypertension are TCM intervention,age,antithrombotic therapy,anti-ischemic myocardial ischemia therapy and ACEI/ARB;related factors of acute heart failure are age,dyslipidemia,anti-myocardial ischemia therapy and lipid-lowering therapy;Cardiogenic shock related factors are TCM intervention,age and dyslipidemia;related factors of severe arrhythmia are TCM intervention and lipid-lowering therapy.The related factors of cardiac death during hospitalization in patients with myocardial infarction complicated with type 2 diabetes were TCM intervention,age,hypertension and ACEI/ARB;the related factors of acute heart failure are age and dyslipidemia;the related factors of cardiogenic shock were age and hypertension;related factors for the composite endpoint of reinfarction and stroke was hypertension;and a related risk factor for severe arrhythmias was lipid-lowering therapy.The related factors of Cardiogenic death during hospitalization in patients with myocardial infarction and dyslipidemia were TCM intervention,age and anti-myocardial ischemia therapy;the related factors of acute heart failure were age,diabetes and antithrombotic therapy;the correlation of cardiogenic shock factors were TCM intervention,age and anti-myocardial ischemia therapy;related factors of serious arrhythmia were TCM intervention and diabetes.ConclusionThe results during the hospital stay indicated that TCM intervention was related to the occurrence of major end points of myocardial infarction.Logistic regression analysis showed that TCM intervention can significantly reduce the incidence of major end points in hospitalization in different subgroups of patients with myocardial infarction.TCM has a significant effect on different patients with myocardial infarction,and provides a basis for the evaluation of the efficacy of traditional Chinese medicine in the treatment of the occurrence of TCM treatment and end point events during follow-up of different subgroups of patients with myocardial infarction,and to explore the correlation about them,and to provide evidence-based medical evidence for the evaluation of clinical efficacy of TCM.MethodsThe questionnaires for patients with myocardial infarction were completed by telephone,outpatient service and home visits.Follow-up records the use of TCM after discharge from the patient and the occurrence of end points.The data was recorded in the Epidata database.SPSS 22.0 and Logistic regression analysis were used to analyze the correlation between TCM interventions and end point events in different subgroups of patients with myocardial infarction.ResultsMultivariate Logistic regression analysis showed that the related factors of Cardiogenic death during follow-up of patients with myocardial infarction and hypertension were TCM intervention,age,diabetes,antithrombotic therapy and lipid-lowering therapy;the relevant factors of recurrent infarction and stroke combined end point events were TCM intervention,gender,diabetes and antithrombotic therapy;related factors of acute heart failure are TCM intervention,age,antithrombotic therapy and lipid-lowering therapy;the relevant factor of severe arrhythmia is age.The related factors of Cardiogenic death during the follow-up of patients with acute heart failure were TCM intervention,antithrombosis therapy and lipid-lowering therapy;Cardiovascular re-admission related factors are TCM intervention,antithrombotic therapy,anti-myocardial antithrombotic therapy;the relevant factors of recurrent infarction and stroke composite end point events were TCM intervention,age,gender and antithrombosis therapy;The relevant factors for acute heart failure are age,diabetes,and antithrombotic therapy;the relevant factors for cardiovascular readmission are gender and antithrombotic therapy.ConclusionThe results of follow-up patients showed that the long-term application of TCM treatment can reduce the incidence of cardiogenic death,heart failure,reinfarction,and stroke after discharged from different subgroups of patients with myocardial infarction,and can significantly improve the symptoms of patients with different myocardial infarction.Improving the quality of life and prolonging the life span provide a basis for the long-term efficacy of TCM in the treatment of myocardial infarction and is worthy of clinical application.
