Objective: To observe the clinical curative effect of Zishen Yangyin Liangxue Prescription combined with small dose hormone on the treatment of liver and kidney yin deficiency type ITP,and its effect on serum cytokines TPO,TNF-?,IL-11 level,and to explore the molecular level mechanism of traditional Chinese medicine in the treatment of ITP.Methods: 40 patients according to the visiting sequence,were randomly divided into hormone group(20 cases)and traditional Chinese medicine + hormone group(20 cases),and randomly selected 18-65 years-old normal people(20 cases)as normal control group to test serum TPO,TNF-?,IL-11.The hormone group was treated with small dose of prednisone,and the traditional Chinese medicine + hormone group was given to Zishen Yangyin Liangxue Prescriptionn combined with small dose of prednisone.After 8 weeks,the efficacy and the levels of serum TPO,TNF-?,IL-11 in the two groups were observed,and the levels of serum TPO,TNF-alpha and IL-11 in the normal control group were detected.Results: 1.In terms of total clinical efficacy,the total effective rate of Chinese medicine + hormone group(75%)was higher than that of hormone group(45%)(P<0.05).2.In terms of efficacy of TCM syndrome,the TCM syndrome score of Chinese medicine + hormone group decreased significantly compared with that of hormone group after treatment(P<0.05),and the total effective rate of Chinese medicine + hormone group (90%)was significantly higher than that of the hormone group(70%)(P<0.05).3.In platelet count,platelet levels were significantly increased in two groups after treatment(P<0.05),and Chinese medicine + hormone group increased more significantly than that in hormone group(P<0.05).4.In the degree of bleeding,the bleeding score of the two groups was significantly decreased(P<0.05),and the degree of bleeding was lower(P<0.05)in the Chinese medicine + hormone group.5.In terms of quality of life,after treatment,the KPS scores of the two groups were improved,in traditional Chinese medicine + hormone group,12 cases(60%)were improved and 7 cases(35%)were stable and 1 cases(5%)were reduced,and in hormone group,5 cases(25%)were improved and 11 cases(55%)were stable and 4 cases were reduced(20%),and the difference was significant(P<0.05).6.In the level of cytokine,two groups of patients before treatment were lower than the average level of TPO,TNF-?,IL-11 levels were higher than that of normal people,both the level of TPO after treatment with different degrees of recovery,TNF-? and IL-11 levels were significantly decreased,and the traditional Chinese medicine + hormone group,TPO,TNF-? level tends to be normal,the adjustment of traditional Chinese medicine + hormone group three cytokines were significantly greater than the hormone group(P<0.05).7.By one element linear regression analysis,there is a correlation between the platelet count and the level of TPO,but the R2 value is too small to explain;and the platelet count is negatively correlated with TNF-? and IL-11.There are correlations between TPO and TNF-? and IL-11,but the R2 value is too small to explain;and the IL-11 is positively correlated with TNF-?.Conclusion:The treatment of ITP with Zishen Yangyin Liangxue Prescription combined with small dose hormone can obviously improve the symptoms of the patients,reduce the bleeding and improve the quality of life.It can also participate in the immune regulation mechanism of the body and regulate the level of ITP related cytokines in order to achieve the purpose of raising the platelet.
