Objective:To investigate the intervention effect of Modified Yangjing Zhongyu Decoction on the ovulation premature in patients with deficiency of kidney essence and Diminished Ovarian Reserve(DOR)during natural cycle in vitro fertilization and embryo transfer(NC-IVF-ET).And to discuss the mechanism of premature ovulation among patients with DOR as well as the possible mechanism of Modified Yangjing Zhongyu Decoction on premature ovulation with DOR.Methods: This study will meet the diagnostic and dialectical standard IVF-ET in patients with deficiency of kidney essence of DOR in 60 patients,30 patients in the observation group entered the NC-IVF-ET treatment cycle after taking a treatment of Modified Yangjing Zhongyu Decoction;Control group without any drug intervention,directly into the NC-IVF-ET treatment cycle.AFC,serum FSH,LH,E2 levels and FSH / LH ratio in the observation group before and after treatment were compared.Comparison of Quantified TCM Syndromes of deficiency of kidney essence before and after Treatment in Observation and Control Groups.After treatment,the levels of AFC,serum FSH,FSH / LH and E2 in observation group and control group on the 3rd day of menstruation werecompared;Observation group and control group,the days of h CG,the dominant follicle diameter,serum LH,E2,P levels;Oviposition rate,excellent egg rate,2PN fertilization rate,excellent embryo rate,pregnancy rate and cycle cancellation rate in observation group and control group were compared.Observation group and control group to cancel the reasons for the cycle;observation group and control group were oocyte follicular fluid Gn SAF,INHB levels;AFC,serum FSH,FSH / LH and E2 levels on the 3rd day of the menstrual period and the age and the years of infertility were compared between egg-received(ER)and no-egg(NE)groups before treatment.ER and NE group,the number of days of h CG menstrual flow,the dominant follicle diameter,the number of dominant follicles and serum LH,E2,P levels;The levels of Gn SAF in follicular fluid Correlation analysis with age,years of infertility,AFC,serum FSH,LH,FSH / LH and E2 levels,as well as the day of h CG,the diameter of dominant follicles,the number of dominant follicles and the serum levels of LH,E2 and P;The levels of INHB in follicular fluid Correlation analysis with age,years of infertility,AFC,serum FSH,LH,FSH / LH and E2 levels,as well as the day of h CG,the diameter of dominant follicles,the number of dominant follicles and the serum levels of LH,E2 and P;Correlation Analysis of Gn SAF and INHB Levels in Follicular Fluid of Ovary.Results:After treatment,the serum basal endocrine FSH level,FSH/LH ratio,and syndrome deficiency of kidney essence deficiency syndrome were significantly lower in the observation group than before(p<0.01).Compared with the control group,serum FSH levels,FSH/LH ratios,TCM syndrome scores,and serum P levels and cycle cancellation rates were lower in the observation group after treatment(p<0.05);the number of menstrual days and the average of the observation group h CG The number of oocytes retrieved,the percentage of eggs obtained and the follicular fluid Gn SAF and INHB were higher than those in the control group(p<0.05).Compared with the non-obtained group,the age of the former group,the level of serum FSH and the level of serum P on h CG day were lower than those of the latter group(p<0.05).The number of days of menstruation and h CG in the oocyte group was higher than that in the non-obtained group.Pearson correlation analysis revealed that follicular fluid Gn SAF and INHB were negatively correlated with the baseline E2,serum HH,E2,P,and dominant follicle diameters(p<0.05 and r<0),respectively;The number of days was positively correlated(p<0.05 and r>0);INHB level in follicular fluid was positively correlated with the average number of oocytes retrieved and follicular fluid Gn SAF levels,respectively(p<0.05 and r>0).Conclusion:Modified Yangjing Zhongyu Decoction can improve the symptoms of patients with deficiency of kidney essence and deficiency of ovarian reserve,and can effectively improve the index of ovarian reserve loss.Yangjueweichen decoction can improve the oocyte recovery rate and reduce the cycle cancellation rate and other aspects of natural periodic renal insufficiency deficiency of ovarian reserve dysfunction in patients with IVF treatment and reduce the natural cycle of renal deficiency patients with deficiency of ovarian reserve dysfunction The incidence of ovulation early.The mechanism that Yangjing Decoction can reduce the incidence of ovulation early may be related to the improvement of deficiency of kidney essence,the improvement of ovarian reserve function,endocrine and autocrine function,the secretion of Gn SAF and INHB in follicular fluid and the inhibition of pituitary Gn secretion.Early ovulation may occur with the symptoms of kidney essence and ovarian reserve dysfunction.
