Objective:To evaluate fetal cerebral blood perfusion with congenital heart diseaseby three-dimensional power doppler and color doppler.Methods:The umbilical artery pulsation index(UA-PI)and the middle cerebral artery pulsation index(MCA-PI)of the fetus were calculated by two-dimensional color doppler ultrasound in 156 fetuses,including 105 normal cases(control group).and 51 fetuses with congenital heart disease(1.transposition of the great arteries group 6 fetuses;2.hypoplastic left heart syndrome group 12 fetuses;3.left sided obstructive lesions group 16 fetuses;4.right sided obstructive lesions group 17 fetuses.).The ratio of MCA-PI to UA-PI was also calculated(CPR= MCA-PI/UA-PI).Three-dimensional power doppler ultrasound combined with virtual organ computer-assisted analysis(VOCAL)technology was utilized simultaneously.In such blood perfusion parameters of the proximal part of the middle cerebral arteryas vascularization index(VI),blood flow index(FI)and vascularization blood flow index(VFI),were calculated to evaluate the fetal cerebral perfusion during pregnancy.Results:Compared with the normal control group,there were a significant increase in MCA-PI,CPR,and VI,FI,and VFI in HLHS fetus and LSOL fetus(P<0.001).However there were no significant differences in perfusion parameters amongthe right heart obstruction fetus,complete aortic transposition fetus,and the normal fetus(P>0.05).Conclusions:Fetal cerebral perfusion with congenital heart disease(CHD)significantly increased,especially in fetuses with HLHS and LSOL.Prenatal application of three-dimensional energy doppler and color doppler can effectively evaluate the fetal cerebral perfusion withthese congenital heart diseases.
