ObjectivesThe aim of present study was to investigate the depression in patients with cervical cancer(CC)during concurrent chemoradiotherapy,evaluate the quality of life,and explore the association between the depression and quality of life in patients with CC to provide a base of clinical nursing intervention.MethodsUsing a convenient sampling method,217 patients with CC hospitalized with concurrent chemoradiotherapy were recruited from three tertiary hospitals at Hefei from 2016-08-01 to 2017-05-30.Beck Depression Inventory(BDI),Quality of life questionnaire-core(EORTC QLQ-30),Family APGAR Index(PAGAR),Social Support Rate Scale(SSRS)and a self-designed general situation questionnaire were used to investigate.Quantitative data were expressed as mean±standard deviation(SD)or media with inter-quartile rang(IQR),and comparisons between groups were performed using t-test,analysis of variance(ANOVA),or Mann-Whitney U test and Kruskal-Wallis H test;Qualitative data were expressed as frequency(n,%),and?~2test was used to perform the comparison between groups.Binary Logistic regression analysis was used to explore risk factors of depression and quality of life in patients with cervical cancer.All data were entered via EpiData 3.1 software and all analyses were conducted by SPSS 16.0 software.ResultsThe mean age of 217 patients was(52.09±9.15)years.Of the patients,98%were married;62.2%were illiterate,25.3%had primary education only,and 12.4%were secondary education and above;72.8%patients with monthly income of less than3000 yuan.Total score of BDI in patients with CC was(20.93±8.81)and the prevalence of depression were 70.5%;score of PAGAR and SSRS(7.75±2.04)and(36.78±6.6),respectively.The results showed that family care,social support,family income,diagnosis grade,age,treatment method and health care number of medical staff were associated with depression of patients.And the score of BDI and SSRS and each dimensions of them were significantly negatively associated with depression with r values ranging from-0.428 to-0.298(P<0.001)and from-0.453 to-0.265(P<0.001),respectively.Results of binary Logistic analysis identified that diagnosis grade was the risk factor of depression while family care and social support was protective factors.Score of overall quality of life were 41.67(8.33),and age,diagnosis grade and treatment method were significantly related to quality of life in patients.There were statistically significant differences of patients at different age groups in the scores of social function in the functional domain,nausea and vomiting dimensions in the symptom domain(P=0.041;P=0.014,respectively).Besides,there were statistically significances in the scores of physical function,role function and social function in the functional domain and fatigue in the symptom domain among cervical cancer patients with different diagnostic stages(P<0.001;P=0.014;P=0.031;P=0.001,respectively).In addition,there were statistically significant differences in the scores of physical function,emotional function and cognitive function in the functional domain,as well as fatigue,nausea and vomiting in the symptom domain among cervical cancer patients with different treatment methods(P=0.025;P=0.044;P=0.044;P=0.018;P=0.040).Furthermore,results of correlation analysis between quality of life and depression indicted subscale of QLQ-30 were significantly related to depression except for three symptoms subscale(dyspnea,constipation and diarrhea).ConclusionsDepression is a common symptom in patients with cervical cancer during concurrent chemoradiotherapy,which is affected by many factors and closely related to patients'quality of life.Early diagnosis,early treatment,increasing the number of health education for patients and improving interventions such as patients'family care and social support to decrease or eliminating depression symptom are important for improving the quality of life of in patients with cervical cancer during concurrent chemoradiotherapy. |
