Clinical Applications Of Tow Methods Of Pancreaticojejunostomy In Pancreaticoduodenectomy

Aim: In 1898,the Italian scholar Codivilla patients for the first time in the block of the pancreaticoduodenectomy(PD).It become the first in the history of in patients with complete resection of Pancreaticoduodenectomy,which begin the history of surgical treatment of pancreatic cancer.At present,there were a lot of the choice of pancreaticointestinal anastomosis,but there is no clear guide to make a unified opinion on the choice of PD,which makes it urgent to find a safe and simple anastomosis.The purpose of this study was to investigate and analyze the clinical effects of pancreatic-jejunal mucosa anastomosis and strap-bound pancreaticointestinal anastomosis in the treatment of pancreaticoduodenectomy.Method: We make a retrospective study of t of 57 cases of pancreaticoduodenectomy in An Hui medical university in the past five years,including 31 cases of pancreatic duct-jejunum mucosal anastomosis type and 26 cases bundled pancreaticoduodenal intussusception anastomosis,Collection patients' age,gender,BMI,preoperative albumin level,preoperative liver function,preoperative complications,operation time,hospital stay the differences between two types of pancreatic anastomosis of two methods were analyzed retrospectively,try for the choice of the ways of pancreatic anastomosis to give some reference opinions and standards.Result: The operation time of bundled pancreaticojejunostomy was shorter than that of pancreaticojejunostomy(358.7 ± 7.8)min vs 307.4 ± 5.8),P < 0.05).There was no significant difference in the main complications of pancreatic fistula,abdominal hemorrhage and hospital stay between the two groups.Conclusion: Pancreatojejunal mucosal anastomosis and bundled pancreaticojejunal intussusception anastomosis have better clinical effects.The operation time of bundled pancreaticojejunal intussusception anastomosis is shorter than that of pancreatic duct-jejunal mucosal anastomosis.The specific anastomosis method is flexibly selected according to individual habits and proficiency.
