| Objective: To investigate the early complications and risk factors after radical cystectomy with orthotopic neobladder,in order to provide a clinical reference for preventing and reducing early complications after radical cystectomy plus orthotopic neobladderMethods: the clinical data of 152 patients undergoing radical cystectomy with orthotopic neobladderfor bladder cancer from January 2010 to December 2016 in our hospital were retrospectively analyzed.The early postoperative complications(< 90 days)were graded according to the Clavien criteria,and the risk factors of common postoperative complications were analyzed by T test,χ2 test and Logistic regression analysis.Results:Of the 152 patients,67 had early complications.The most common complications were infection related complications,gastrointestinal tract related complications and incision related complications.Through univariate analysis,patients with BMI(Body Mass Index),preoperative comorbidity score(CCI),operative time,intraoperative blood loss,and surgical approach may be the influencing factors for early postoperative complications.Logistic multivariate regression analysis was used to analyze the data that may be the influencing factors.The results showed that the patients had a BMI(OR=1.071,P=0.045)preoperative comorbidity score CCI(OR=1.012,P=0.000),and the operation time(OR=1.065).P=0.018)and intraoperative blood loss(OR=1.404,P=0.003)were significantly associated with postoperative complications.Conclusion:The incidence of early complications after radical cystectomy with orthotopic neobladder was higher in order of infection-related complications,gastrointestinal related complications and incision-related complications.Multivariate logistic regression analysis showed that BMI,preoperative comorbidity score CCI,operative time,and intraoperative blood loss were risk factors for early postoperative complications after in situ cystectomy.Before the operation of bladder cancer patients,we should select the patients strictly,strengthen the management of the patients with higher BMI,correct the complications actively before operation,control the related complications,operate carefully during the operation,reduce the bleeding during the operation,and shorten the operation time.It is helpful to reduce the incidence of early postoperative complications. |
