A Clinical Study About The Effects Of Clear Aligners On Periodontal Indexes And Supragingival Plaque Microbes

Objective:The real-time quantitative PCR?qPCR?technique was used to detect the changes of streptococcus mutants?S.mutans?and Porphyromonas gingivalis?P.gingivalis?in patients with dental plaque in the oral cavity at the pre-treatment?T₀?,1 months after?T₁?and 3 months after?T₂?.Examined and recorded the periodontal clinical indexes at the three periods.Then to evaluated the condition of the patients'periodontal health.Furthermore,the effect of aligners on periodontal health was studied.Methods:15-patients with clear aligners were selected in the clinical before the start of the experiment.At the T₀?beginning of the treatment?,T₁?1-month?and T₂?3 months?,the periodontal clinical indexes of the patient's Ramfjord teeth were examined and recorded that including the plaque index?PI?,gingival index?GI?,bleeding on probing?BOP?,and probing depth?PD?.In each of these three periods,the plaque in the mouth of the subject was collected by using a sterile periodontal curettage and placed in an EP tube which containing 1 ml of sterile PBS?PH=7.4?.Before the bacterial genomic DNA extracted,the mixture was shaked and stored in-20°refrigerator.The study used qPCR real-time fluorescence quantitative to detect and quantitate the number of S.mutans and P.gingivalis in each sample.According to the standard curve,to work out the bacteria copies and obtain the percentage of S.mutans and P.gingivalis in total bacteria at T₀,T₁ and T₂.Then compared the changes of bacteria in three periods.Results:No changes in PD were observed,but scores for BOP,PI and GI increased after clear aligners placement?P<0.05?.Compared with T₀,S.mutans and P.gingivalis were elevated at T₁ and T₂,and the difference was statistically significant.In three periods,S.mutans was detected and increased.There were 2 cases that did not detect P.gingivalis at the T₀,but T₁ and T₂ were both detected,and the content was adding.Conclusion:As with other aligners,clear aligners can affect periodontal health and promote the accumulation of the S.mutans and P.gingivalis on the plaque in the short-term.The study indicating that doctors should pay close attention to the changes of oral hygiene in the process of clinical use of therapy.
