Objective:To investigate whether there is abnormal expression of miR-34 a in patients with depressive disorder,and whether there are differences in the expression of mi R-34 a between patients with depressive disorder and clinical characteristics,and the relationship between the expression of miR-34 a and the efficacy of depressive disorder.Methods:A case-control study was conducted in which 82 patients with depressive disorders in the outpatient and inpatient departments of the First Hospital of Shanxi Medical University were recruited in strict accordance with exclusion criteria.Informed consent was signed for82 patients and 60 controls.All patients undergoing SCID screening met the DSM-IV diagnostic criteria for depressive disorder.The Hamilton Depression Scale(HAMD17)and the Hamilton Anxiety Scale(HAMA)were used to assess the depression and anxiety symptoms of the patients.The Chinese version of beck scate for suicide ideation(BSI-CV)was used to assess the suicidal ideation.The life event scale(LES)and the Childhood Trauma Questionnaire(CTQ)were used to assess the environmental aspects of life events.All subjects took 4ml fasting cubital venous blood and extracted RNA.The expression level of miR-34 a was detected by Quantitative real-time PCR(qRT-PCR).The patients with depressive disorder were analyzed and compared.The expression of miR-34 a in the peripheral blood of the control group,the expression of miR-34 a in patients with depressive disorder of different clinical characteristics,and the diagnostic efficacy of miR-34 a in depressive disorders.To further understand the relationship between miR-34 a and depressive disorder,patients with depressive disorder were treated with standard systemic therapy for 8 weeks.The expression level of miR-34 a was reexamined at the end of the 8th week,and the clinical efficacy was evaluated.The treatment of patients with depressive disorder was analyzed and compared.Differences in the expression of miR-34 a and the relationship between miR-34 a expression and the efficacy of depressive disorder.Results:1.The expression of miR-34 a was significantly increased in the case group(Z=-7.009,P=0.001).ROC curve analysis showed that AUC for mi R-34 a expression was 0.849(95%CI=0.787–0.911)with a sensitivity of 76.80% and a specificity of 85.00%.The expression of miR-34 a was lower in patients with stress-induced depression than in those without stress(Z=2.205,P=0.043).2.The expression of miR-34 a in patients with depressive disorder was significantly lower after treatment(Z=-2.140,P=0.032),but it had not returned to normal levels.There was no statistically significant difference between clinical remission group and clinical non-remission group(P>0.05).There was no statistically significant difference between response patients and non-response patients(P>0.05).Conclusion:1.Abnormal expression of miR-34 a in the peripheral blood of patients with depressive disorder is associated with the occurrence of depressive disorder and the presence or absence of stress.The miR-34 a expression level may have a good predictive value for the diagnosis of depressive disorder.2.The expression of miR-34 a in patients with depressive disorder was reduced after antidepressant treatment,but the expression level of miR-34 a has not been shown to correlate with the efficacy of depressive disorder.
