Objective: The purpose of this study was to investigate the difference of vascular complications between early-onset diabetes(EOD)and late-onset diabetes(LOD)of type 2 diabetes mellitus.Methods:After matching age and sex by 1:2 by propensity score matching method in EOD group and LOD group.A total of 780 T2DM(male: 47.7 %,mean age 53.32 ±6.18 years)were included when the onset age was defined by 40 years old.A total of 537 T2DM(male: 49.9%,mean age 75.26 ±3.83 years)were included when the onset age was divided into 60 years old.Macrovascular complications include cardiovascular diseases(left ventricular diastolic dysfunction(LVDD),left ventricular remodeling(LVR),carotid atherosclerosis(CAS))and lower extremity atherosclerosis(LEA).Microvascular complications include diabetic retinopathy(DR),chronic kidney disease(CKD)and peripheral neuropathy(DPN).The odds ratio of vascular complications in EOD and lod was obtained by logistic regression analysis.Results: In all patients,the relevance rate of microvascular complications(DR?CKD?DPN)was 18.4% and23.6%,24.8%,respectively;and the microvascular complications had(LVDD?LVR?CAS?LEA)was 70% and 47.7%,29.3%,41.4%,respectively.After matching for sex and age,and adjustment for HbA1c%,insulin,oral hypoglycemic,hypotensive,lipid-lowering drugs,etc,there was no significant difference between the early onset group and the late onset group in macrovascular complications whenever the onset age was divided into 40 or 60 years old.Compared with late-onset diabetes,when the onset age was divided into 40 years old,the OR(95%CI)of microvascular complications(DR?CKD?DPN)in patients with early-onset diabetes was 2.906(1.972-4.282),P=0.000 and 1.967(1.387-2.791),P=0.000;1.672(1.152-2.429),P=0.007.When the divided onset age was 60,the OR(95%CI)of DR and DPN was 1.763(1.092-2.847),P=0.020 and 1.675(1.106-2.537),P=0.015;while there was no obvious difference in CKD(P>0.05).Conclusion: The risk of microvascular complications was not increased in patients with type 2 diabetes mellitus,but there was more risk of microvascular complications.
