?Objective?To explore the relationship between the different imaging chracteristics and the clinical datas and to analysis related factors,to analysis the related factors about prognosis in the middle cerebral artery territory cerebral infarction.?Methods? In this retrospective analysis,we investigated patients that visited to one hospital from September 2014 to December 2016.All patients were diagnosed as cerebral infarction of middle cerebral artery territory.We devided the patients into two different groups according to their imaging characters and m RS scores.One group contained the middle cerebral artery territory,the upper branch and the down branch,the other group contained good prognosis and poor prognosis.We collected the main datas :Demographic information:gender,age;Past history: hypertension,diabetes,stroke/TIA,atrial fibrillation;NIHSS scores,m RS scores;Homocysteine(HCY),Low-density lipoprotein(LDL),D-Dimer,Fibrinogen,Hb A1 c,fasting blood glucose;bilateral carotid intima-media thickness(IMT),Microbleeds Positive rate,Deep myeloid veins expansion,the stenosis rate of M1 in cerebral Infarction of middle cerebral artery territory.we made a comparison of the imaging characters and other datas of the middle cerebral artery territory group,the upper branch group and the down branch group;and a comparison of the datas of good prognosis and poor prognosis.We finally explored the related risk factors in the cerebral infarction of the middle cerebral artery territory.?Results?(1)The NIHSS scores(9.35±7.93),blood HCY concentration and the stenosis rate of M1 in cerebral Infarction of middle cerebral artery territor In the cerebral Infarction of middle cerebral artery territory were both higher than the other two groups,and the difference was significant(F=5.19,p=0.01;F=4.16,p=0.02 ??2=18.34,p=0.00).While differences in the other indexes were not significant.(2)The age in the poor prognosis group(66.96±11.11)was higher than the good prognosisgroup(60.10±11.10),the difference was significant(F=3.15,p=0.00).The NIHSS scores(8.93±6.99)was also higher than the other one(3.44±2.78),and the difference was also significant(F=5.18,p=0.00).What's more,the coefficient of variation of Systolic blood pressure(8.69±3.46)were also higher than the other one(6.78±2.09),and the difference was also significant(F=2.63,p=0.01).(3)Compared with the good prognosis group,the poor prognosis group was middle cerebral artery territory(?2=8.68,p=0.00),and in this group the CMBs often positive(?2=5.46,p=0.02),DMV degree was often >1(?2=6.14,p=0.01).(4)According to the logistic regression analysis,the age(OR=1.06,p=0.02,95%CI : 1.01,1.11),NIHSS scores(OR=1.31,p=0.03,95%CI : 1.12,1.53),24 h variation of Systolic blood pressure(OR=1.31,p=0.00,95%CI:1.12,1.53),DMV degree>1(OR=0.27,p=0.02,95%CI:0.09,0.80),CMBs postive(OR=3.58,p=0.03,95%CI:1.12,11.47)were the risk factors which could lead to the poor prognosis in the cerebral infarction of the middle cerebral artery territory.?Conclusion?1.The degree of atherosclerosis in large artery group of middle cerebral artery territory is more serious than the upper group and the down group,and the mechanism is related with atherosclerosis.2.The aged,high NIHSS degree,high coefficient of variation of Systolic blood pressure,DMV grades>1,CMBs positive are the independent risk factors in the cerebral infarction of the middle cerebral artery territory.
