| Objective:To explore the relationship between TCM syndrome elements and cardiac function classification in patients with chronic heart failure,to provide objective and standardized basis for TCM syndrome differentiation of chronic heart failure,and at the same time to provide reference for clinical treatment of chronic heart failure with integrated traditional Chinese and western medicine.Methods:Information collection is carried out in a retrospective way,and inpatients in the emergency department of affiliated hospital of Chengdu university of traditional Chinese medicine from December 2015 to December 2017 are selected as the research objects,and patients who meet the diagnostic criteria of chronic heart failure and whose heart function is class ii-iv,except exclusion criteria.According to the contents of medical records,basic information of patients(name,sex,age,primary disease),four diagnostic materials of traditional Chinese medicine,syndrome differentiation,nyha classification,LVEF value,nt-pro BNP and other information are collected,at least one doctor at or above the attending level participates in the re-determination of TCM syndromes of the research object,and the syndrome type is decomposed into evidential elements.Set up a database of the above data,and use Chinese version of SPs 20.0 software for statistical analysis.Results:1.The prevalence rate of chronic heart failure is the highest among 70-79 age groups.According to the statistics of observation indicators,coronary heart disease accounted for the highest percentage,followed by hypertension,cor pulmonale and rheumatic heart disease.The patients with heart failure were mainly cardiac function class iii,accounting for 55.48 %.2.There were significant differences in cardiac function classification and nt-pro BNP distribution among CHF patients at different ages(p < 0.05).3.The frequency of disease elements from high to low is qi deficiency,blood stasis,phlegm,yang deficiency,water drinking and yin deficiency,and the frequency of disease elements from high to low is heart,lung,kidney,spleen and liver.Qi deficiency + blood stasis is the most common combination of two disease elements,while heart + lung is the most common combination of two disease elements.4.There is statistical difference in the distribution of disease elements in different cardiac function grades in patients with heart failure,and there is statistical difference in the distribution of disease elements in different cardiac function grades in patients with heart failure(p < 0.05).5.There was statistically significant difference between the distribution of disease elements and disease location elements in patients with heart failure(p < 0.05).6.There was significant difference between CHF patients’ cardiac function classification and lev value,nt-pro BNP(p < 0.05).Conclusion:1.Patients with chronic heart failure are mainly elderly,coronary heart disease is the main basic disease of chronic heart failure,and inpatients are mainly cardiac function class iii.2.There is correlation between TCM syndrome elements of heart failure and cardiac function classification.Heart function class ii → class iii → class iv,the syndrome elements of disease type change from qi deficiency to yang deficiency,and the syndrome elements of disease type change from heart,lung to kidney.Qi deficiency and blood stasis run through the disease all the time,and the pathological products are phlegm and water.qi deficiency,blood stasis and water stoppage are the key pathogenesis of chronic heart failure.3.The higher the cardiac function series,the more yang deficiency syndrome and water-drinking syndrome are increasing,which indicates that these two syndrome elements are indicative signs of deterioration of cardiac function.At the same time,the proportion of positive disease elements increases,so at the same time of treating the root causes of late heart failure,we need to take into account phlegm,drink,blood stasis and other related pathological products,and treat both the symptoms and rootcauses.4.LVEF and NT-proBNP are correlated with cardiac function classification,which can be used as reference for cardiac function classification. |
