Conventional Ultrasound Combined With Elastography Area Ratio In Breast BI-RADS 4 Application Of Pathological Diagnosis

Abstract/Summary:
Objective to investigate the conventional ultrasound method and the elasticit y imaging in breast area ratio application value of BI-RADS4 type lesions.The union of the conventional ultrasound and elasticity imaging method to measure the breast lumps line lesion(A surface)on the maximum transverse diameter section,after the largest diameter section(B),(C),on the top and bottom diameter section is used to analyze the elastic imaging image,determine different aspects in breast BI-RADS4 type of pathological changes in the actual value.Methods 96 patients with breast lump d iagnosed by conventional ultrasound method and diagnosed as BI-RADS 4 level in our Hospital from from March 2015 to December 2016 were selected,and there were a total of 115 lesions.The breast lumps line lesion(A surface)on the maximum transverse diame ter section,after the largest diameter section(B),(C),on the top and bottom diameter section,and observe the form,boundary,with or not with calcification,the distribution and resisitance of bloodstream and so on.when measured,the probe is as vertical as the bump,let the bump was located the center of the image,hold on the probe,switch to the model of UE/US.Make sure the image of UE,the image is at least 2 times as much as the bump.The machine calculation the ratio of acreage,the A,B,C,and recorded.The diagnostic criteria of the union is the same result of the three surface as the final result.Analyze the differed surface in breast BI-RADS4 type of pathological changes in the actual value.Results Among the 115 breast masses,82 were benign and the ratio is 71.30;33 were malignant,with ratio is 28.70.Among the benign,37 were fibroadenoma of breast,15 were adenosis of mammary glands,6 were adenosis of mammary glands with fibroma,10 were sclerosing adenosis of the breast,9 were intraductal papiloma of breast,3 were granuloma and 2 were galactocele.Among the malignant,22 were infitrating ductal carcinoma,5 were colloid carcinoma,3 were cancer in situ,1 was malignant phyllodes tumor,1 was small cell carcinoma.The elastosonography ratio and area ratio of three groups had statistical difference(P<0.05).The critical point of A,B,C are 1.32、1.26、1.53.In the sensitivity,the union > B>C>A,with no statistical difference(P>0.05).The ratio of malignant was higher than benign,The union can raise accuracy.Conclusions Ultrasound elastography can indirectly reflect the area ratio of breast tumor and surrounding tissue involvement degree,BI-RADS in distinguishing 4 types of benign and malignant breast tumors,A,B,C three different sections of conventional ultrasound combined with elastography area ratio method sensitivity,specificity and accuracy were high.The three section combined diagnosis,to identify benign and malignant tumor and improve the diagnostic sensitivity,accuracy and positive predictive value has important significance,can be used to determine the BI-RADS of 4 types of benign and malignant masses in the preferred method,it is worthy of further promotion in the clinical application.
