Atrial fibrillation(AF)is the most common arrhythmia at present,with age the prevalence rate is increasesing,Atrial fibrillation and heart failure(HF)are often merged,The two are reciprocal cause and effec,Atrial fibrillation occurs in the ventricular rate increased and ventricular rhythm is absolutely not neat,atrioventricular dyssynchrony,atrial contraction dysfunction,which decreased cardiac output and rapidly caused by neurohormonal activation,thereby promoting the occurrence of heart failure.Heart failure can make atrial fibrosis and abnormal regulation of ion channels,neurohormonal activation,atrial filling pressures,further increase of atrial fibrillation.Objective:To explore the risk factors for patients with atrial fibrillation and heart failure and to provide effective prevention and prevention measures for heart failure which were caused by atrial fibrillation.Methods:A total of 142 with atrial fibrillation in Chengde Medical College were enrolled,from January 2013 to December 2017,divided into two groups with Whether or not have heart failure :79 patients A group(without heart failure),63 patients B group(heart failure group).Statisticsed the general clinical data of two groups : Sex,age,history of hypertension,history of diabetes,the typ of atrial fibrillation;Related inspection data: echocardiography:Internal diameter of left atrium,left ventricular ejection fraction;24 h Holter monitors index:Mean ventricular rate.biochemical markers:uric acid.The single factor analysis was used to analyze the general clinical data and related examination data of two groups,Incorporating statistically significant correlation indices into multiple factor Logistic regression to Further screening the risk factors of patients with atrial fibrillation and heart failureResults:1 General clinical data analysis showed that the gender,age,hypertension,diabetes and the typ of atrial fibrillation was no significant difference in two groups.the difference was no statistically significant(P>0.05).2 In the two groups,there was a significant difference of left atrial size(P < 0.05),combined The heart failure group Left atrial diameter is greater than that of the without heart failure group.3 In the two groups,There was a significant difference of 24 h Holter monitors index,(P < 0.05).4 In the two groups,There was a no significant difference of biochemical markers of uric acid,(P>0.05).5 The statistically significant correlation indices were analyzed by logistic regression analysis showed that left atrium diameter greater than 42 mm,the average heart rate is greater than or equal to 90 / min,are independent risk factors of atrial fibrillation in patients with heart failure.Conclusions:1 echocardiographic index of left atrium diameter greater than or equal to 42 mm is the independent risk factors of atrial fibrillation in patients with heart failure.The corresponding intervention measures should be taken to prevent the occurrence of malignant events2 24 h Holter monitors index of the average heart rate greater than or equal to 90 / min,are independent risk factors of atrial fibrillation in patients with heart failure.The ventricular rate should be actively controlled to prevent the deterioration of cardiac function in patients with atrial fibrillation.
