Objective: to explore the application of multichannel microelectrode recording in the surgical treatment by deep brain stimulation for Parkinson's disease(PD)and to analyze the clinical effect of multichannel microelectrode recoding by comparing to that of single channel microelectrode recoding.Method: 75 Parkinson's disease patients,including 42 males and 33 females who came to the department of neurosurgery,Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology,Wuhan,China received deep brain stimulation operation from September,2014 to September 2016 was retrospectively observed and analyzed.These patients are average aged at 64.3 +3.4 years old(variation from 50 years old to 74 years old)and have 150 microelectrodes transplanted into bilateral subthalamic nucleus(STN)in total.Among these 150 microelectrodes recording,100 of which were single channel microelectrode recoding and 50 of which were multichannel microelectrode recoding.We collect the clinical motor function scores applied to estimate movement according to the UPDRS-III scoring system for these 75 patients after opening of the microelectrodes and the minimum effective dose patient needed to control PD symptom pre-and post-surgery,and analyze the difference of motor function scores and minimum effective dose between pre-and post-surgery,and between patients who received multichannel microelectrode and who received single channel microelectrode by software SPSS 20.0,and difference is considered significant when the P<0.05.Result: all of these 75 patients have satisfactory periodical fellow up record which contains 6 to 24 months'(average duration is 16 months')clinical information.Except 4 patients found to have intracranial pneumatosis,none of these 75 patients suffered from complication such as intracranial hemorrhage,infection,Skin damage ect,and all of them had improvement in motor function and good recovery.Specially,patients who received multichannel microelectrode,although had no significant difference in improving motor fuction compared to that of the single channel microelectrode(p>0.05),but they had a more Significant motor symptom(p<0.05)and a less dose of anti-Parkinson's disease drug need(p<0.05)than that of pre-operation.Conclusion: In terms of ameliorating Parkinson's disease motor obstacle symptom,multichannel microelectrode recoding has no significant difference with single channel microelectrode recording,reduces postoperative dose and multichannel microelectrode recoding can be applied to clinical functional neurosurgery since it will not increase surgical complication.
