Study On Public Reporting Systems Of Nursing Care Facilities

[Objective] Our study aims to develop the framework of public reporting system with the guidance of American nursing care system and experience and to provide policy suggestion for nursing facilities in ChinaBased on retrospective literature review,DADS model and information transmission theory were applied to analyse the relevant nursing care practice and provide basis for developing the theory of public reporting of nursing care by focus group.And the components and relationship between components were evaluated.Finally,model of public reporting of nursing care practice was framed.[Methods]By objective and systematic analysis the data collected from governmental websites,the public reporting of nursing care practice in America has been explored in detail.The results of the main component,operation and influencing factors guided the further analysis.By retracting data from website,75 nursing care facilities in 16 provinces with certain selective criteria,the contents,quality and ways of public reporting has been summarized.The PEST analysis method was applied to compare the political,economic,social and technological environment between Chinese and American public reporting of nursing care.[Results]Theoretical framework of nursing care facilities(NCFs)was constituted by subject,content,analysis,distribution,governing and object of public reporting of information.Information was generated and analyzed by subjects,then released and distributed to objects via public reporting channels in an appropriate way.During the process,governing is applied to ensure the quality and effect.Regarding to the public reporting systems of NCFs in the U.S.,the objects are diversified organizations led by governments.They first collect data from standardized and complete data source,then analyze accessed data using risk adjustment and ranking transformation,and distribute information to objects via website named “Nursing Home Compare” and “Nursing Home Initiative”.The reasons for why public reporting systems functioning well of NCFs are due to 1)scientific,standardized system regarding to disclosure content which ensured the value of disclosed information;2)the governing of process of public reporting also contributes to the functioning of public reporting system.As a typical representative of NCFs in China,the subject of nursing homes are themselves and they released information at their official websites.Currently,there are many problems of public reporting of NCFs in China,including lacking standards of released content,poor quality of reported information,simple method for public reporting and absence of information regarding to quality of care.In addition,no organization was responsible for governing.By comparing the experience of public reporting in NCFs between China and the U.S.,there are issues needed to improve regarding to public reporting system in China,including subject,content,quality,channels,types and governing.From the angle of external environment and based on analysis of social-economic factors,the construction and improvement of public reporting system of NCFs could significantly contribute to meeting the requirement form consumers/patients,improving the quality of care,reducing the occurrence of adverse events of nursing services,increasing the rational use of health resources and boosting the efficacy of health services.From the political and technological perspective,there is no supporting environment regarding to public reporting of NCFs in China.One is the lacking of law or rules which lead to unclear of what content should be public reported and confusion that who shares the responsibility of governing;the other is due to the slow construction of information system of NCFs in China in addition to the lacking of effective assessment tools which results in the limitation of formation of public reporting system.[Conclusion and suggestion] For the inner system perspective,the subject,content and public surveillance of public reporting existed some questions,and it also lacked good policies and technology from outside perspective.Therefore,the public reporting of nursing care in China should be strengthened from the following two parts.Initially,the development of public reporting system of nursing practice should be modified further,covering from diversified subject of public reporting with government as the leading role and set up the comprehensive system for public reporting content,as well as surveillance system.Secondly,improving the outside environment has also been raised,which include strengthening the fiscal input and exploring the mode of insurance combined with nursing care facilities,as well as the construction of information system.
