Objective: A systematic review and Meta analysis of ultrasound-guided transversus abdominis plane block for postoperative analgesia in gynecological laparotomy,in order to have a better understanding of the safety and efficacy in clinical application.Methods: Using the search strategies,we searched Pubmed,Embase,the Cochrane Library;Chinese databases such as CNKI,CBMdisc,VIP database,Wanfang database for clinical trials contain ultrasound-guided transversus abdominis plane block for postoperative analgesia in gynecological laparotomy.The quality of each included RCT was assessed according to the Cochrane Handbook 5.0.The mean differences(MDs)and 95%confidence intervals(CIs)were chosen to present the continuous outcomes,as to dichotomous outcomes,the odds ratios(ORs)and 95%CIs were used.Results: The Meta analysis of the thirteen selected trials was carried out and the results were as follows1.Analgesic effect: ultrasound-guided TAP block can significantly alleviate the pain after gynecological laparotomy,and reduce the consumption of analgesics obviously.But the long-term efficacy need to be discussed.2.Side effects: There were no statistical differences in postoperative Ramsay sedation score and nausea and vomiting events between the two groups,and TAP block related adverse events did not occur in either group.Conclusion: Ultrasound-guided TAP block can relieve the short-term pain after gynecological laparotomy,while the long-term analgesic effect may be general.With the addition of high safety,it should be recommended. |
