| Backgroud and object:With the rapid development of minimally invasive technology,most of the diseases in Department of Urology can be operated under endoscope,and compared with traditional operation,there are advantages such as clear field,less trauma,less bleeding and quick recovery.At present,minimally invasive surgery in the Department of Urology can be divided into three major categories: laparoscopic surgery,transurethral surgery and percutaneous nephroscope surgery.Among them,transurethral surgery using natural cavities makes surgery further develop to minimally invasive techniques,mainly for the treatment of benign prostatic hyperplasia,bladder tumors,cystitis glandularis,bladder stones,ureteral calculus,renal calculus,urethral strictures and so on,and most cases are the first choice.With the popularization of transurethral surgery,the research and development of its related instruments are also on the rise.From the earliest electric knife,pneumatic and so on,to the present plasma ring electrode,green laser,holmium laser,thulium laser,plasma pestle electrode and so on.We use the latest application of plasma needle electrode in transurethral surgery,to explore the efficacy and safety of transurethral plasmakinetic needle electrode enucleation of prostate and plasmakinetic needle electrode en-bloc transurethral resection of bladder tumor.Methods:Methods 81 cases of benign prostatic hyperplasia and bladder tumor collected from Tianjin Medical University General Hospital in recent 1 years.There were 36 cases of benign prostatic hyperplasia and 45 cases of bladder tumor.They were treated by plasma needle electrode and thulium laser.Physiological saline was applied to the flushing fluid during the operation.We recorded the operative time,amount of flushing fluid,difference of hemoglobin and hematocrit before and after operation,time of stopping irrigation after operation,time of catheter removal after operation(except for bladder tumor patients),and the postoperative complications.The two groups were compared with the same disease,carry out statistical data processing,and summarize the practical experience of the two kinds of operation in the same operator.Comprehensive analysis and comparison of the safety and efficacy of the two types of operation.Results:1?All patients completed the operation successfully under general anesthesia,The difference of hemoglobin and hematocrit before and after operation in the experimental group of patients with benign prostatic hyperplasia and bladder tumor were less than that of the control group(P<0.05).The time of urethral catheter removal in the experimental group of benign prostatic hyperplasia was lower than that of the control group(P<0.05).2?Follow-up of a month after operation,there was no significant difference in postoperative complications between the experimental group and the control group of the patients with benign prostatic hyperplasia and bladder tumor.3?Actual operation experience during the operation,the plasma needle electrode is more tactile than thulium laser,the operation is controlled more easily,the directivity is more clear.Conclusion:1?Plasma needle electrode is the most frontiers treatment in the operation of transurethral surgery in Department of urology,there are no relevant reports at home and abroad,and it is worth further exploring.2?The plasma needle electrode is more tactile than thulium laser,the anatomic level is clearer,the directivity is more clear,the hemostatic effect is better and the operation is controlled more easily in the transurethral enucleation of prostate and En-bloc transurethral resection of bladder lesions.It is helpful for the rapid recovery of patients after transurethral enucleation of prostate.The plasma needle electrode can avoid the injury of adjacent important tissues and organs during the operation,it is particularly suitable for operative region requiring fine operation.It is indicated that the plasma needle electrode has high safety and good curative effect. |
