Objective: To investigate the relationship between fibrosis score and cardiac structure and function in patients with type 2 diabetes mellitus with nonalcoholic fatty liver disease. Methods: This study was retrospective. Objective To select 243 T2 DM patients with non-alcoholic fatty liver(NAFLD)admitted to the Department of Endocrinology at the Third Central Hospital of Tianjin within the past two years. All data of patients' general data, biochemical indicators, liver ultrasound, and cardiac ultrasound were collected for statistical analysis.Among them,32 patients underwent liver biopsy.According to the NAS stage,it was divided into 18 cases without fibrosis group and set as group A.The fibrosis group consisted of 14 patients and was designated as group B.Analyze and compare the difference of biochemical indexes, cardiac function and structure between the two groups, and analyze the related factors of fibrosis regression. The patients were calculated according to the formula nonalcoholic fatty liver fibrosis score(NFS)and then grouped according to the score. If NFS?-1.455,progression of hepatic fibrosis in NAFLD can be ruled out and a total of 98 patients in group C were included.NFS? 0.676 was considered to have progressive hepatic fibrosis in NAFLD and was defined as 69 patients in group E.Between the two groups, not only NAFLD hepatic fibrosis was defined as 76 cases in group D.Baseline data, blood glucose and blood lipids, uric acid, insulin resistance and cardiac function and structure were compared among the three groups, and related statistical analysis was performed.Risk factors for diastolic function and ventricular hypertrophy were analyzed by regression. Results: The results showed that group B was significantly higher in age,HOMAIR and NFS than Group A,and the LFC level of Group A was significantly higher than Group B,the difference was statistically significant(p<0.05),Group B LVD,IVST,LVM,LVMW were significantly higher than group A,and Group B e/ A significantly lower than Group A(P0.05)regression analysis showed that HOMA-IR and NFS scores were risk factors for hepatic fibrosis.Grouped by NFS score the results showed:(1)There was no statistically significant difference in age, sex, history of diabetes, height, hip circumference,waist-hip ratio and diastolic blood pressure among three groups.(2)The RBC,HDL-C and AST were not statistically significant among group C,Group D and Group E three.WBC and CCR were statistically significant between groups C and E,there was no statistically significant difference between P<0.05,C group and D Group,Group D and Group E.TG and LDL-C in three groups were higher than normal value, the three groups showed a gradual rise in the trend, there is a statistically significant P0.05.ALT,UACR and FFA showed an increasing trend in the three groups, and Group E was statistically significant compared with Group C and Group D,while Group C and D had no statistical significance.(3)The indexes of Group C,Group D and Group Three compared FPG,0.5hPG,1hPG,2hPG,3hPG,ins0.5h,ins1 h,INS2H,ins3 h,cp1h,cp2 h and cp3 h were not statistically significant. There was a trend of HbA1 c and cp0.5h between three groups, and Group E had statistical significance compared with Group C and Group D,P<0.05,C Group had no statistical significance. The Group E was significantly higher than the Group C and D groups in the comparison between the three groups, and the HOMA-IR was statistically significant and P0.05.The fins,FCP and LFC of the three groups were gradually increasing, and the comparison between the three groups was statistically significant,P0.05.(4)There were no statistically significant indices of left atrium, left ventricular posterior wall, right atrium, right ventricle and LVEF between three groups. In Group E and Group C,the left ventricle significantly increased and the LVM was in Group C,which was statistically significant and P<0.05.In Group C,Group D and Group E Three,LVMW showed an increasing trend,e/a showed a trend of decreasing gradually, which was statistically significant and P0.05.(5)Two logistic regression analyses showed that BMI and uric acid were risk factors for NFS scoring, age, history of diabetes,TG,LDL-C,and NFS were the risk factors for cardiac hypertrophy, the history of diabetes mellitus and the risk factors for the evaluation of NFS were divided into left ventricular diastolic dysfunction.
