Objective:Retrospective analysis of 16488 cases of gastric cancer biological characteristics and surgical treatment,in order to further improve the surgical treatment of gastric cancer provide the basis.Methods:Collected 16,488 patients who underwent surgery from 1966 to2015.Grouping criteria:A time period(1966-1975),B time period(1976-1985),C time period(1986-1995),D time period(1996-2005),E time period(2006-2015).To analyze the data of five periods of total,early and advanced gastric cancer patients: 1.Biological characteristics of gastric cancer;2.Surgical treatment.Using SPSS21.0 software,statistical analysis of the data,P<0.05 showed a statistically significant difference.Results:1.Biological characteristics of gastric cancer1)Gender:Overall,early and advanced gastric cancer men were higher than women,but their composition ratio did not change significantly in 50 years.2)Age:The average age of gastric cancer,early and advanced gastric cancer has been on the rise for 50 years.3)Tumor site:Overall gastric cancer,early and advanced gastric cancer tumors showed a trend of upward shift in 50 years.4)Tumor size:The tumor size of overall and advanced gastric cancer is mainly≥5cm,but its composition ratio has declined over the past 50 years.Early gastric cancer is mainly <5cm,and its composition ratio is on the rise.5)Borrmann classification:In the past 50 years,Borrmann I and II have shown an upward trend,Type III has shown a downward trend,and Types IVand V have not changed significantly.6)Infiltration depth:The total gastric cancer and advanced gastric cancer were mainly T4,but their composition ratio showed a downward trend in the past 50 years,and the overall gastric cancer T1 showed an upward trend.7)Lymph node metastasis:The lymph node metastasis rate of total gastric cancer and advanced gastric cancer were 61.2% and 65.6% respectively,but the lymph node metastasis rate in the first 50 years showed a trend of first increase and then decrease.The rate of lymph node metastasis in early gastric cancer was 10.1%,and it showed a downward trend.8)TNM stage:Over the past 50 years,overall gastric cancer and advanced gastric cancer have shown a downward trend in phase IV,with an upward trend in phase I-III.Early phase I gastric cancer showed an upward trend.9)Differentiation:Overall gastric cancer,early stage and advanced stage gastric cancer are mainly moderately-differentiated type,and their composition ratio has been on an upward trend for 50 years.10)The incidence of early gastric cancer was 7.96%,showing an upward trend in 50 years;advanced gastric cancer was 92.04%,showing a downward trend in 50 years.2.Surgical treatmentThe radical resection of overall gastric cancer and advanced gastric cancer has been on the rise since the last 50 years.Palliative resection,reduction tumor resection,and simple exploration have shown a downward trend.Conclusion:1.There is no significant change in the composition ratio of males and females.2.The average age shows an upward trend.3.The tumor’s predilection site showed a trend of upward shift.4.The long diameter of the tumor in patients with overall and advanced gastric cancer is mainly ≥ 5cm,while that of early gastric cancer is mainly<5cm.5.Borrmann type I and type II showed an upward trend,type III showed a decreasing trend,Borrmann type IV and V type had no significant changes.6.T1 and T2 in patients with gastric cancer and advanced gastric cancer showed an increasing trend,and T4 showed a decreasing trend.Currently,T4 is predominant.Patients with early gastric cancer are all T1.7.In general gastric cancer and advanced gastric cancer,lymph node metastasis is the main,showing a decreasing trend.Patients with early gastric cancer are mainly without lymph node metastasis,showing an increasing trend.8.Overall gastric cancer and advanced gastric cancer patients showed a downward trend in phase IV,with phase I,phase II,and phase III showing an upward trend.Patients with early gastric cancer are mainly in stage I,showing an increasing trend.9.The degree of tumor differentiation was mainly in the moderately-differentiated type and showed an upward trend.10.The incidence of early gastric cancer was 7.96%,showing an upward trend in 50 years;advanced gastric cancer was 92.04%,showing a downward trend in 50 years.11.Radical resection in patients with general gastric cancer and advanced gastric cancer showed an upward trend.
