Objective:The objective of this study is to investigate the clinical application value of conventional ultrasound,virtual touch tissue quantification ultrasonic imaging technology(virtual touch tissues quantification,VTQ)in identification of chronic hepatitis with liver fibrosis caused by various reasons(This study mainly focused on chronic hepatitis B)Materials and methods:In this study,totally 112 patients suffering from chronic hepatitis with liver fibrosis who were tested the elasticity of liver tissue and determined by percutaneous liver biopsy in our hospital from Jul 2016 to Aug 2017 were analyzed retrospectively.Before clinical treatment,the results of conventional ultrasound(two-dimensional ultrasound/color Doppler ultrasound)and VTQ in patients was observed,and the diagnostic results were compared with pathological results in order to investigate the clinical value of different ultrasonic diagnosis technologies in liver fibrosis by various causes,so as to provide rational guidance to clinical treatment.Results:1.In these 112 patients with chronic hepatitis with liver fibrosis,there were 86 Surface antigen positive hepatitis b,4 autoimmune hepatitis,2 hepatitis C,6 Primary biliary cirrhosis,6 steatohepatitis,2 Alcoholic hepatitis,4 drug induced liver disease,1 schistosomiasis and 1 Glycogen liver disease.2.Conventional ultrasound image features for chronic hepatitis with liver fibrosis :(1)typical conventional ultrasound image features for fatty liver patients: the echo of the liver is(2)attenuated;conventional ultrasound image features for patients with chronic hepatitis: typical liver echodense thickening under uniform or uneven;(3)conventional ultrasound image features for patients with liver cirrhosis: typical liver echodense thickening uneven accompanied with nodular changes,disorders of intrahepatic structure,vascular and support structure of display is not clear,liver capsule was not smooth(jagged or wavy).The difference between the conventional ultrasound and pathological cases was statistically significant(P<0.05).3.There was significant statistical difference(P<0.05)at the quantitative ultrasound imaging and pathological cases for different degree of liver fibrosis.4.The sensitivity,specificity and accuracy for diagnosis of chronic hepatitis with hepatic fibrosis by conventional ultrasound were 68.66%,52.63% and 65.12% respectively;while these values examined by VTQ were 70.15%,47.37% and 68.12% respectively.Conclusions:1.Conventional ultrasound is currently the most common determination method for chronic hepatitis with hepatic fibrosis.It is easy and convenient,and has specific signs for the diagnosis of liver fibrosis caused by various reasons,however with large overlap.2.As a new perspective and method for the grading of liver fibrosis,VTQ not only provides very important information for clinical treatment,but also becomes the supplement and development of conventional ultrasound.The pathological examination results are the final diagnosis standard.The combination of VTQ and pathological examination results has good clinical value to evaluate the degree of liver fibrosis.3.Effective combination of conventional ultrasound and VTQ can improve the accuracy of ultrasonic diagnosis of chronic hepatitis with liver fibrosis.VTQ can provide assistance to increase the correlation and consistency of the fibrosis grading grading made by pathological diagnosis,which is a sensitive method for detecting chronic hepatitis with liver fibrosis,as well as cirrhosis.
