Objective: To investigate the relationship between Prognostic Nutritional Index(PNI)and clinicopathological features and prognosis in patients with colorectal cancer.Methods: Collect the clinical data of patients with pathologically confirmed colorectal cancer who underwent radical resection of colorectal cancer in the First Affiliated Hospital of China Medical University from November 2012 to July2015.PNI was calculated based on the blood test and biochemical test of the first examination after admitted to the hospital.The PNI was calculated as follows:albumin+5×total lymphocyte count.According to the results of PNI,patients were divided into high PNI group(PNI?45)and low PNI group(PNI <45).The survival curve was plotted by kaplan-meier method,and the difference between the two groups was analyzed by log-rank test.Cox proportional risk regression model was used to analyze the clinicopathological features and prognosis of PNI and colorectal cancer patients by single factor and multifactor analysis.Results:A total of 193 patients with colorectal cancer were enrolled in this study.All patients had a minimum PNI of 34.5,a maximum of 72.4 and an average of 51.3 ±5.9.There was no significant difference in gender and age between patients with high PNI and low PNI(P> 0.05).In these two groups,there were significant differences between PNI and tumor location(P = 0.030),tumor diameter(P = 0.014),tumor invasion depth(P = 0.026),lymph node metastasis(P = 0.010),TNM stage(P =0.004),distant metastasis(P=0.046),CEA(P = 0.012)and CA199(P = 0.020),but not with the degree of tumor differentiation,tumor type(P> 0.05).Univariate analysis showed that PNI(P = 0.001),tumor differentiation(P<0.001),tumor gross classification(P = 0.006),tumor invasion depth(P = 0.004),lymph node metastasis(P<0.001),TNM stage(P<0.001),distant metastasis(P<0.001),CEA(P= 0.002)and CA199(P<0.001)were closely related to the survival of patients with colorectal cancer.Multivariate analysis showed that only PNI(P = 0.025),tumor differentiation(P<0.001),distant metastasis(P<0.001)and CA199(P<0.001)were independent prognostic factors for survival of patients with colorectal cancer.Conclusion: 1.PNI was significantly associated with tumor location,tumor diameter,tumor invasion depth,lymph node metastasis,TNM stage,distant metastasis,preoperative CEA and CA199 in patients with colorectal cancer.2.PNI is an independent factor affecting the prognosis of patients with colorectal cancer.
