Objectives To explore the effect of noninvasive ventilator in lung cancer after operation.Further analysis of smoking factors,different surgical procedures and surgical resection scope were performed.Methods The Department of thoracic surgery of the Affiliated Hospital of North China University of science and technology was selected for admission to the hospital for pulmonary disease,and the surgical treatment was required.The patients were diagnosed as lung cancer by rapid pathological diagnosis,including 80 patients,including 43 male patients,37 female patients and 42 years old ~71 years(55.92 + 6.45).The eligible patients were randomly divided into the conventional treatment group(group A)and the noninvasive respiratory unit(group B).The conventional treatment group was treated by closed thoracic drainage,anti infection,oxygen inhalation,expectorant,atomization and nutritional support after operation.Noninvasive respiratory unit treatment plan is based on conventional treatment,postoperative non-invasive ventilator assisted ventilation for 24 hours.We should improve the grouping of experimental research and collect data according to pre designed experimental ideas.For the surgical patients,the patient's sex,age,smoking history,preoperative examination,preoperative pulmonary function,arterial blood gas analysis,hypersensitive C reaction protein(Hs-CRP),calcitonin(PCT),erythrocyte sedimentation rate(ESR),and chest CT were used.Set the experimental observation time point,take 12 hours,24 hours and 48 hours after the operation as the observation point,take the whole blood of arterial blood,detect the Pa02/ PaC02 value of arterial blood gas analysis,compare the value of Pa02/ PaC02 in the same time period of the two groups.In the first day,third days and seventh days after the operation,the venous blood was taken to detect the hypersensitive C reaction protein(Hs-CRP),calcitonin(PCT)and erythrocyte sedimentation rate(ESR),and the changes of the inflammatory factors in the two groups were compared and analyzed.The chest DR was reviewed to know whether the lungs recovered after operation,whether there was inflammatory exudation and lung tissue expansion.Results Compared with the routine treatment group,the difference of Pa02,Hs-CRP,PCT and ESR after the operation of non invasive breathing unit were statistically significant,and the difference of PaCO2 value at 12 h and 24 h points was statistically significant,and there was no statistical significance at the time point of 48 h.Stratified according to smoking,repeated measurements of Pa02/ PaCO2,Hs-CRP,PCT,ESR value,each observation index had statistical significance at different time points.There was no difference between PaO2 groups in smoking patients,P=0.055>0.05,no statistical significance;PaCO2,Hs-CRP,PCT,ESR groups were statistically significant;PaO2 at different time points and group interaction test Smoking and non-invasive ventilator interact with PaO2 at the time of observation.The values of Pa02/ PaCO2,Hs-CRP,PCT and ESR were repeated according to the method of operation.The statistical significance of each observation index at different time points were statistically significant.The interaction between the different time points of PaO2 and the group interaction test(thoracoscopic operation F=1.601,P=0.202>0.05),and(F=1.992,P=0.128>0.05)were not found.There was interaction between PaO2 and non-invasive ventilator at the time of observation.The values of PaCO2,Hs-CRP,PCT and ESR have statistical significance at different time points,and all of them have statistical significance.The interaction between different time points and groups of PaO2(F=2.231,P=0.099),(F=1.039,P=0.367>0.05)in different surgical excision ranges(open surgery F=1.039,P=0.367>0.05)is not found in the observation of different surgical excision ranges and non-invasive ventilator.The interposition has interaction with PaO2.Conclusions For patients with lung cancer,the use of noninvasive ventilator after operation has good effect.It can better improve the patient's ventilation and be beneficial to the recovery of postoperative patients.For patients with history of smoking,the shortterm application of non-invasive ventilator was not obvious after the operation;no matter the effect of thoracoscopic surgery or thoracotomy,the use of non-invasive ventilator had good therapeutic effect on different range of excision.
