ObjectiveTo investigate the value of coaxial needle in Cone-Beam Computed Tomography(CBCT)guided lung puncture biopsy and analyze the related factors of complications.MethodsA total of 256 people in our hospital from April 2015 to August 2016 had pulmonary lesions were retrospectively analysis,divided into 2 groups,each group had 128 patients.Single needle puncture biopsy was defined as group A,and coaxial needle puncture biopsy as group B,the diagnostic sensitivity,specificity,accuracy and the occurrence of common complications were compared between the 2 groups.ResultsThe diagnostic sensitivity of group A was 87.3%(96/110)and 96.3%(105/109)of group B,the chi-square test showed statistical difference(c2=5.954,P=0.024).The diagnostic specificity of group A was 100%(18/18)and 100%(19/19)of group B,the two groups had the same diagnostic specificity.The diagnostic accuracy of group A was 89.1%(114/128)and 96.9%(124/128)of group B,the chi-square test showed statistical difference(c2=5.976,P=0.025).The incidence of complications in group A was 14.1%(18/128),21.9%(28/128)in group B,there was no statistical difference in chi square test(c2=2.650,P=0.142).CBCT guided lung biopsy technique success rate was 100%(256/256),pathological diagnosis accuracy rate was 93.0%(238/256),complication rate was 18.0%(46/256),and pneumothorax rate was 8.2%(21/256),pulmonary hemorrhage rate was 9.77%(25/256).The chi-square test showed statistically significant differences(P<0.05)between the sexes,puncture depth,distance away from the pleura,and whether there had emphysema of the groups.There were statistically significant differences between lung hemorrhage in lesion size,puncture depth,distance away from the pleura,and the number of biopsy specimens acquired(P<0.05).Logistic regression analysis showed that male and emphysema were risk factors for pneumothorax.The risk factors of pulmonary hemorrhage were the long puncture depth,small lesion,and biopsy specimens more than one time.ConclusionCBCT-guided lung coaxial puncture biopsy can improve the sensitivity and accuracy of disease diagnosis.Male,long puncture depth,emphysema,small lesion,and biopsy specimens more than one time are the risk factors for CBCT-guided lung puncture biopsy.
