Background and Objective Human Immunodeficiency Virus(HIV)is the pathogen of acquired immune syndrome(Acquired Immunodeficiency Syndrome,AIDS).With the promotion of the combination of high performance antiretroviral therapy(highly active antiretroviral therapy),the virus has been well controlled and further monitored.Measuring the efficacy of antiviral therapy is the key to ensuring success.The prophase group independently developed the HIV antibody against HIV p24 and gp41 antibody(luciferase immunosorbent assay,LISA),which is more sensitive and more quantitative than the conventional enzyme linked immunosorbent assay(ELISA).In this study,the antibody of serum samples before and after HAART in 75 cases of HIV infection was detected.It was found that the serum anti HIV p24 antibody and gp41 antibody in the sera of the patients decreased in varying degrees,and the decline was parallel to the decrease of HIV virus load and negative correlation with the rise of CD4 cells,and the HIV antibody in HIV infected persons was further analyzed.The effect of LISA system on the monitoring methods of anti HIV efficacy was evaluated.At the same time,combined high performance antiretroviral therapy can effectively control HIV replication,prolong the survival time and reduce the mortality of HIV/AIDS patients.But because of the existence of the HIV-1 virus reservoirs,the HIV can not be eradicated,and there is no way to quantify the size of the HIV virus library accurately,but the research found that the HIV-1 antibody are associated with the virus reservoirs.This is the basis for the study of the correlation between HIV-1 antibody quantification and HIV virus reservoirs.Methods The first part of this study was based on 75 HIV patients in a short-term treatment cohort of AIDS functional cure in Guangzhou,which collected detailed clinical data,such as sex,age,infection pathway,WHO CDC staging,peripheral blood HIV load,CD4+T cell,CD8+T cell count,and HIV-1 subtype detection.LISA was used to detect the anti HIV-1 p24 and gp41 antibody levels at 3,6,12,18 and 24 months during HAART.The changes of the above two antibodies in the HAART treatment and the correlation with the changes in the clinical efficacy and the influencing factors were analyzed.In the second part,the PMBC specimens of 52 HIV infected persons were collected,and 52 patients of Total HIV-1 DNA level at 3,6,12,18 and 24 months during HAART were detected by Realtime polymerase chain reaction,and to explore the relationship between anti HIV-1 p24 and gp41 antibody levels and Total HIV-1 DNA in HAART process.At the same time,the Total HIV-1 DNA results of samples with viral load below the detection limit were selected to analyze the correlation between the level of anti HIV-1 p24 and gp41 antibody and the virus reservoirs.Part ?:Application of HIV-1 antibody quantitative detection in the efficacy evaluation of highly active antiretroviral therapyResults 1.Before and after HAART,the levels of anti HIV-1 p24 and gp41 antibodies in HIV-1 infected patients show a downward trend,and the difference was statistically significant.2.There was a significant positive correlation between the changes of anti HIV-1 p24 and gp41 antibodies before and after HAART in HIV-1 infected patients.3.HIV diagnosis of different intervals of treatment were statistically significant in the change of HIV antibody level.4.There was a significant negative correlation between the anti p24 or gp41 HIV antibody inhibition rate and HIV-1 viral load in HIV-1 infected people before and after HAART,and there was a positive correlation with the CD4+T lymphocyte counts,and all of them were statistically significant.Conclusions 1.Before and after HAART treatment,the decreasing trend of p24 and gp41 antibody detected by LISA method was positively correlated with the decrease of HIV-1 load and the increase of CD4+T lymphocyte,and the change of antibody before and after HAART to some extent could be used to evaluate the therapeutic effect of HIV-1 infection.Part ?:Correlation study between quantitative detection of HIV-1 antibody and HIV-1 reservoirsResults 1.After HAART,the total HIV-1 DNA level was negatively correlated with the CD4+T lymphocyte count,and there was no correlation between the total HIV-1 DNA and the quantitative detection level of the anti HIV-1 p24 antibody,but there was a weak correlation with the trend of the anti HIV-1 gp41 antibody level.However,the levels of anti HIV-1 p24 and gp41 antibodies were not correlated with the virus reservoirs.Conclusions 1.During HAART,the level of anti HIV-1 gp41 antibody was related to the size of Total HIV-1 DNA,but the level of anti HIV-1 p24 and gp41 antibodies can not assess the size of the virus reservoirs.
