SUBJECT BACKGROUND:Pneumoconiosis is a kind of occupational disease,and its cause is very clear.Although scholars at home and abroad exploring continously,the disease is still can not be cured now.In 1980s,mason was the first one to create the large volume of whole lung lavage and achieved a certain curative effect.Our country start to apply this technology to treat pneumoconiosis and clinical research in 1986.With the continuous deepening of the research,the various molecular part of the mechanisms of pneumoconiosis had been revealed.lv lin[45]pointed out that the stages of pneumoconiosis relate obviously to dust exposure time,the dust concentration and the dust capacity.There is a clear correlation between the serum antioxidant and pneumoconiosis staging,because O2-which produced body affected by external factors can be removed by SOD.Through this mechanism,SOD can protect cells from damage.Superoxide dismutase is a protective factor,and it may be reduced by the disease in the early stage.With the continuous inhalation of dust,free radicals in the body will continue to producing,the body increase the level of SOD to compensatory protection,so that the body of the oxidant/antioxidant system to maintain a dynamic balance.This study uses 60 cases underwent whole lung lavage before and after symptoms,pulmonary function,arterial blood gas analysis,SOD and mda levels compared to whole lung lavage in pneumoconiosis patients with stage0+?I,SOD,MDA levels and curative effect.Research methods:In our hospital from january 2013-january 2016 and admitted to the department of thoracic surgery,department of occupation of pneumoconiosis patients,a total of60 cases,all patients met the diagnostic criteria of pneumoconiosis,the study inclusion criteria and whole lung lavage in patients with surgical indications,general anesthesia after whole lung lavage,preoperative and postoperative patients with general symptoms,were recorded respectively.The pulmonary function,arterial blood gas analysis results of SOD and MDA levels were to be analysed.Result:In 60 patients cases,there are welders'pneumoconiosis 35 persons,silicosis pneumoconiosis 15 persons,coal pneumoconiosis 9 persons.Before and after bilateral whole lung lavage at the same period,there are different degrees of improvement of sputum,dyspnea,chest pain and other symptoms,so as the pulmonary function.Compared with before treatment,there was statistical significance of VC,FVC,FVC%,FEV1.0,MVV%?p<0.05?,but exclusively?p>0.05?.And PaCO2,PaO2,MDA,SOD were also significant?p<0.05?.Comparison of average ash powder and free SiO2 in two groups were also significant?p<0.05?.There was no significant difference between the total number of cells?p>0.05?.Conclusion:Large volume whole lung lavage can remove dust,silica and macrophages from pneumoconiosis,and reduce the burden of dust in the lungs.The lung function,VC,FVC,FVC%,FEV1.0 and MVV%were improved after large volume whole lung lavage?WLL?,but the other indexes showed no obvious change.The arterial blood gas analysis was improved after large volume whole lung lavage.the partial pressure of oxygen increased and the partial pressure of carbon dioxide decreased.Large volume whole lung lavage?WLL?can improve serum SOD and reduce MDA in patients.It shows that whole lung lavage?wll?can improve the patients ability to scavenge oxygen free radicals in a certain extent.
