The etiology of vitiligo is very complex.Sincenow,it has been found that some biomarkers are related with activity of vitiligo.These biomarkers can give some hints to the pathophysiology of vitiligo,besides,some of which are very important in monitoring fluctuation of the disease,allowing an early and accurate determination of therapeutic regimen and evaluation of treatment response.Firstly,We are determined to discuss the link between active vitiligo and histopathology of skin biomarkers,circulating biomarkers.Secondly,we are intended to validate the exact sensitivity and specificity of a special biomarker.All studies about biomarkers in skin(n=52)and in sera(n=45),using various of methods to quantify values of those biomarkers,are included according to the predefined inclusion criteria.Unfortornately,analyses about sensitivity and specificity are rare.Spongiosis,epidermal lymphocytosis,basal cell vacuolation,dermal lymphocytes infiltration are relatively effective in reflecting the disease activity of vitiligo,besides,vitiligo histological scoring system could further help determine the activity of vitiligo.If the score exceeds 3,it tends to be unstable vitilgo(sensitivity=80%and specificity=67%).Recently,studies found that CXCL9 were considered to be the promising histological biomarker which could demonstrate possibly better association with progressive disease(sensitivity=100%and specificity=93%).As to circulating biomarkers,it has been found that CD4~+/CD8~+ratio,quatity of Tregs,cytokines(IL-1??IL-10?IL-17and IFN-?),soluble CDs(sCD25 and sCD27),chemokines(CXCL9 and CXCL10),autoantibodies and oxidative stress markers are still under debate and could not be used in clinical preferably.A large multicenter study could make more definitive statements regarding their sensitivity and specificity.
