Objectives The aim of this study is to observe the characteristics of carotid atherosclerosis plaque by the method of High Resolution Magnetic Resonance Imaging(HR-MRI)technology,and compare the stablizing effect for the carotid plaque between the two treatment groups PAS group(probucol,atorvastatin and aspirin)and AS group(atorvastatin and aspirin),to provide more effective solutions for the prevention and treatment of carotid artery plaque.Methods 33 cases with carotid atherosclerotic plaques detected by ultrasonography were enrolled in this study.These patients were either outpatients or hospitalized patients from the neurology department of tangshan workers hospital which is affiliated to North China University of Science and Technology from December 2016 to August 2017,and were randomized into PAS group(16)and AS group(17)for 6 months treatment.The patients in PAS group were treated with probucol(Qilu Pharmaceutical Company Limted,commodity name zhile,0.125g/piece)0.5g/times,orally,2/day,atorvastatin(Pfizer Inc,commodity name Lipitor,20mg/piece)20mg/times,orally,1/day and aspirin(Bayer,Germany,commodity name bayaspirin,100mg/piece)100mg/times,orally,1/day,while patients in AS group were given atorvastatin and aspirin(medicine and usage are the same).At beginning of the study and after 6 months treatment,all patients were given serum tests,such as blood lipid levels including total cholesterol(CHOL),triglyceride(TG),low-density lipoprotein(LDL-C),high-density lipoprotein(HDL-C),and erythrocyte sedimentation rate(ESR),hypersensitive C reactive protein(hs-CRP),to compare the changes of CAP length,thickness,stability and carotid stenosis rate before and after treatment by the method of HR-MRI examination.The two groups of patients were tested for the liver and kidney function in January and June after taking the medicine,and the adverse drug reactions were detected.In order to evaluate the therapeutic effects of the two methods on serum lipid levels,serum inflammatory factors,plaque size,stability and carotid artery stenosis rate in patients with CAP.Results 1.In this study,33 patients were collected,16 in the PAS group and 17 in the AS group,1 case was performed carotid endarterectomy and 1 case was lost in the PAS group,in the AS group 1 case was discontinued and 1 case was lost,so they were withdrew from this study,at last there were 14 cases in the PAS group and 15 in the AS group.There were no statistically significant differences between the two groups in terms of sex ratio,age,previous history,blood lipid level,ESR,hs-crp and CAP morphology(P>0.05).No drug sideeffects were observed in all patients during the treatment.2.The serum levels of CHOL,TG and LDL-C were decreased after treatment in both groups,and the difference was statistically significant(P<0.05),there was no significant change in HDL-C level after treatment in both groups(P>0.05).The reduction rate of the serum levels of CHOL and LDL-C was lager in the PAS group than that in the AS group,and the difference was statistically significant(P<0.05),however,there was no significant decrease in the TG level between the two groups(P>0.05).3.The serum levels of ESR and hs-CRP were decreased after treatment in both groups,and the difference was statistically significant(P<0.05).The reduction rate of the serum levels of ESR and hsCRP was lager in the PAS group than that in the AS group,and the difference was statistically significant(P<0.05).4.The levels of CAP length,thickness and carotid stenosis rate were decreased after treatment in both groups,and the difference was statistically significant(P<0.05).Although the proportion of unstable plaques was reduced in PAS and AS groups,the difference was not statistically significant due to small sample size and short time,etc.(P>0.05).The reduction rate of the levels of CAP length,thickness and carotid stenosis rate was lager in the PAS group than that in the AS group,and the difference was statistically significant(P<0.05).Conclusions 1.PAS and AS can reduce the levels of CHOL,LDL-C,ESR and hs-CRP in patients with carotid atherosclerosis,and the effect of PAS is better than that of AS.2.PAS and AS can lower plaque length,thickness and the stenosis rate of carotid artery in patients with carotid atherosclerosis,but the effect of PAS is better than AS.3.The PAS method has a stronger effect on lowering blood lipid level and anti-inflammatory and anti oxidation.It can be more effective in shrinking CAP and reducing the stenosis rate of carotid artery,and has a clinical application value in anti AS of carotid artery.
