| Objectives:1.How gait speed contributes to knee gaitogram parameters still remains unknown and therefore the current study was designed to evaluate the influence of gait speed on kinematic parameters in healthy subjects.2.Knee osteoarthritis(KOA)is reported to have significantly different characteristic kinematics during walking.However,the relationship between Kellgrence-Lawrence Score(K/L)and knee gaitgoram of patients with medial KOA remains unclear.Hence the aim of the study is to explore the relationship of K/L socore and knee gaitogram of patients with medial KOA.Methods:1.Kinematic parameters of key events of knees(thirty people,sixty knees)at1.0km/h,1.5km/h,2.0km/h,2.5km/h,3.0km/h,3.5km/h and 4km/h were carried out and evaluated knee gaitogram including knee flexion/extension,adduction/abduction,internal/external rotation,anterior/posterior translation,distal/proximal translation,and medial/lateral translation.One-way ANOVA,Pearson correlation and the stepwise regression analysis were conducted in the results.2.Ninety-seven patients with medial KOA and thirty-eight asymptomatic participants were involved.Patients with medial KOA were divided into early,moderate and severe KOA based on the K/L score.Results:1.Significant differences were found between gait speed and twenty-two knee gaitogram parameters at 1.0 to 4.0km/h.At 1.0 to 2.5km/h,eleven knee gaitogram parameters were correlated with gait speed.At 2.5 to 4km/h,only four knee gaitogram parameters were correlated with gait speed.In the order of 1.0 to 4.0km/h,1.0 to 2.5km/h,and 2.5 to 4.0 km/h,the stepwise regression analysis revealed that gait speedcontributed less to knee gaitogram parameters.2.Through the kinematic gait analysis,we find a relationship between K/L score and 3D kinematic gait of patients with medial KOA.All KOA knees had a significantly reduced range of motion.As the K/L score was upgraded,the knee flexion at heel strike and 50% stance phase increased while the peak knee flexion in swing phase decreased;the adduction increased and the femoral rotation increased internally at heel strike,50% stance phase and maximum rotation in the swing phase.Femoral translation increased anteriorly and distally at heel strike and 50% stance phase.The severe OA group had more medial translation than asymptomatic groups.Conclusion:1.Our findings revealed that gait speed was correlated with and contributed to knee gaitogram parameters(1.3 to 32.2%).Besides,knee gaitogram parameters at higher gait speed than self-selected gait speed are suggested to be more reliable than the other speed.The contributions of gait speed to knee gaitogram parameters may be divided into three sub-factors including alteration of gait speed beyond self-selected speed,alteration of gait speed below self-selected speed and range of speed.Contributions increase as an order of alteration of gait speed beyond self-selected speed,alteration of gait speed below self-selected speed and range of speed.2.Significant alterations of three-dimensional joint kinematics were identified in subjects suffering various severities of OA in Chinese patients.This study provides an important reference for the options of treatment,assessment of the therapy and rehabilitation of KOA. |
