Background and Aimobjective:Retrospectively study the different blood usage condition before and after the application of autologous blood transfusion in Old patients with spinal degenerative disease,to investigate the applications values of autologous blood transfusion.Experimental methods and results Method:1.Collected the cases of old patients who take the spine fusion surgery from Jan.2008-Dec.2016.Study the use condition of allogenic blood before and after we use the device of autologous blood transfusion.Divide all the patients into three groups by the time we use autologous blood transfusion device:No use of autologous group(Jan.2008.-Jun.2010.)only use pre-operation autologous group(Jul.2010.-Dec.2014.),Use pre-operation and post-operation autologous group(Jan.2015.-Dec.2016.),study the blood use condition of three groups.2.Study the blood use rate of three groups,the blood transfusion volume of all the patients who use pre-operation autologous blood transfusion and the blood transfusion volume of post-operation autologous blood and the relationship of post-operation autologous blood transfusion and patients condition.3.Divide all the cases into 4 groups by the blood transfusion condition: allogenic blood group,pre-operation autologous blood group,post-operation autologous blood group and pre&post-operation autologous blood group,study the blood RT?coagulation RT and adverse reaction after blood transfusion of each group.Result:1.Normal condition of three groups have no significantly statistical differences,the allogenic blood use rate is 27.3% pro-operation and 20.4% pre-operation in the no use of autologous group,the allogenic blood use rate is 2.3% pro-operation and 13.8% post-operation in the only use pre-operation autologous group,the allogenic blood use rate is 4.8% pro-operation and 3.2% post-operation in the Use pre-operation and post-operation autologous groups.The statistical results showed significant statistical difference(P<0.05),this result indicate autologous blood transfusion could obviously decrease the allogenic blood use,especially when combined the pre-operation and post-operation method.2.The blood transfusion rate of three groups showed no statistics difference(P>0.05),in the patients who take the usage of intraoperation autologous blood transfusion devices,the effective rate(>200ml as useful)is 95% and in the patients who take the postoperation autologous blood transfusion devices,the effective rate(>200ml)is 76%.correlation analysis showed the intra&post-operation blood loss associated with the operation time and fusion levels.This result indicate the indication of autologus blood transfusion is well controlled.but this result still inidicated the post-operation autologus blood transfusion is not so easy control.3.the blood RT?coagulation RT of three groups had no significantly statistical differences in the 4 groups by the blood transfusion condition(P>0.05),this result indicate the autolgous blood transfusion have the some effect with the allogenic blood transfusion.the autologus blood transfusion could reduce some adverse reaction after blood transfusion(hypothermia& rash,P<0.05).Conclusion:1.the clinical application of autologous blood transfusion in the intraoperation is:more than 2 levels fusion and estimated operation time more than 2 hours.In the postoperation condition,the application of autologous blood transfusion is: more than 2 levels fusion,operation time more than 2 hours and hemorrhage during operation>400ml.2.Intra&post-operation autologous blood transfusion could reduce the use of allogenic blood use3.Intra&post-operation autologous blood transfusion is safety and effective,worth clinical promotion.
