Objectives:To study the surgical treatment and postoperative complications of posterior fossa medulloblastoma tumors and to improve the clinical understanding of posterior fossa medulloblastoma. Methods:Surgical treatment, postoperative complications and treatment measures of 25 cases of posterior cranial medulloblastoma in Department of Neurosurgery,First Affiliated Hospital of Kunming Medical University from 2012 to 2017 were retrospectively analyzed. The data was analyzed by SPSS 17.0.Results:All 25 patients were resected after midline approach.Seven patients had obstructive hydrocephalus with ventriculo-peritoneal shunt,4 patients had preoperative(cerebellar surgery)ventriculo-peritoneal shunt, and 3 patients(cerebellar surgery)performed ventriculo-peritoneal shunt. All shunt had good effect.Pathological examination of 25 patients showed 20 cases of medulloblastoma and 5 cases of desmoplastic.Four patients had subcutaneous effusions, which were completely absorbed by subcutaneous effusion after pressure dressing and subcutaneous effusion drainage. One case of Bacterial culture of subcutaneous effusion/cerebrospinal fluid was Acinetobacter baumannii, and Acinetobacter baumannii positives used antibiotics based on drug susceptibility results and had negative etiological tests after treatment.Ten patients with persistent fever after cerebellar surgery(T>38?,continued for more than 3 days),3 patients were treated with antibiotics(1 case of scalp effusion bacterial culture suggested Acinetobacter baumannii positive,1 case had pulmonary infection,1 cases with persistent body temperature>39? and symptomatic treatment was not effective);10 patients underwent lumbar puncture(4-7 days, once daily),and no bacterial growth was observed in cerebrospinal fluid culture.10 cases of patients with fever after treatment were restored to the normal temperature. Conclusions:The medulloblastoma has its unique clinical features.Microsurgery is an important method to treat medulloblastoma.Lumbar puncture is an effective method to diagnose and treat postoperative fever.
