Objective:To investigate and analyze the the data of intraoperative and postoperative pancreatoduodenectomy,compare the curative effect of pylorus preserving pancreaticoduodenectomy and traditional pancreaticoduodenectomy.Methods:the clinical data of 63 patients with malignant tumors around the head and ampulla of the pancreas from December 2015 to November 2017 were retrospectively analyzed.The surgical methods were divided into the traditional Whipple group(n=31)and the pylorus preserving pancreatoduodenectomy(Pylorus-preserving Pancreaticoduodenectomy,PPPD)group(n=32).Statistics of the general clinical data,sex,age,liver function,complication(hypertension,diabetes,coronary heart disease),using SPSS 21 software analysis,P>0.05,the general data of each group is not statistically significant,with comparability.Statistical and postoperative data,surgery(operation time,intraoperative bleeding),postoperative hospital stay,3 days after operation,changes in liver function,early postoperative complications(biliary fistula,pancreatic fistula,postoperative bleeding,gastric emptying delay),postoperative long-term complications(dumping syndrome,camp barrier,alkaline reflux gastritis),postoperative The data of gastric tube time,whole fluid diet time and normal diet time were retained.T test and X2 test were used by SPSS 21 software analysis.If P<0.05,the difference was statistically significant.If P>0.05,the difference was not statistically significant.Results:There were 63 cases of successful operation and no death cases.The operation time,the time after the operation,the time of gastric tube retention,the whole fluid diet time,the recovery of normal diet time,and the early postoperative complications(delayed gastric emptying),the long term complications(dumping syndrome,nutritional barrier and alkaline reflux gastritis)in the PPPD group were P<0.05 compared with the Whipple group,and the difference was statistically significant.3 days after operation,glutamic pyruvic aminotransferase(ALT),3 days after operation,gluten transaminase(AST),total bilirubin(TBil),and intraoperative hemorrhage in 3 days after operation were compared with Whipple group P>0.05,the difference was not statistically significant.Group PPPD and group Whipple had early postoperative complications(biliary,pancreatic fistula,postoperative infection,postoperative bleeding)P>0.05,the difference was not statistically significant.Conclusions:Pylorus preserving pancreatoduodenectomy has no significant difference in the rate of bleeding and early postoperative complications(bile,pancreatic fistula,postoperative bleeding,postoperative infection)compared to the traditional pancreatoduodenectomy,but shortening the operation time and simplifying the operation,preserving the pylorus and vagus plexus,and preserving at the same time The digestive and storage functions of the stomach have greatly improved the quality of life in the early and long term.It is more conducive to the recovery of the patients after the operation,the time of hospitalization is short,the pain of the patients is reduced,and the patient is recovered quickly.Therefore,pylorus pancreatoduodenectomy is still worthy of clinical promotion under strict control of surgical indications and radical operation.
