Objective:To investigate the expression of candidate tumor suppressor genes SASH1,E-cadherin,p53 and BRAF V600E mutation in thyroid papillary carcinoma,and their relationship with clinical parameters,and try to explore their role in the pathogenesis of papillary thyroid carcinoma.Methods:In the first part,59 cases of thyroid papillary carcinoma were selected,and the expression of SASH1,E-cadherin and p53 in thyroid papillary carcinoma cells and paracancerous cells was observed by immunohistochemical staining.The mean optical density of E-cadherin protein expressed in cytoplasmic membrane and SASH1 protein expressed in cytoplasmic was recorded,and the number of positive cells expressing p53 in nucleus was recorded.The relationship between the optical density of SASH1,the mean light density of E-cadherin and the positive rate of p53 and the clinicopathological parameters are measured by SPSS.In the second part,36 patients were selected to amplify the DNA of tumor cells by polymerase chain reaction(Polymerase Chain Reaction,PCR),and the mutation rate of BRAF gene was detected.The relationship with the clinical case parameters and SASH1,p53,E-cadherin and BRAF V600E were analyzed.Results:In 59 cases,the mean optical density of cancer cells of SASH1 and E-cadherin are both lower than that of the adjacent normal cell,the difference was statistically significant(p<0.05).The positive rate of p53 was 24%.The mutation rate of BRAF is 80.6%The mean optical density of SASH1 has no correlation with gender,age,tumor size,serum TSH,lymph node metastasis,TNM stage(p>0.05).The mean optical density of multifocal papillary thyroid carcinoma is lower than that in solitary papillary thyroid carcinoma,the difference was statistically significant(p<0.05).Binary logistic analysis showed that the mean optical density of SASH1 is a protective factor for multifocal thyroid papillary carcinoma,and the size of tumor is a risk factor.The mean optical density of E-cadherin is correlated with age and TSH level,but has no correlation with gender,tumor size,serum TSH,lymph node metastasis,TNM stage,moltifocal or that of SASH1.There was a higher mean optical density in cancer cells of patients older than 45 years old,the difference was statistically significant(p<0.05).Binary logistic analysis showed that the mean optical density of E-cadherin is a protective factor for lymph node metastasis,and the size of tumor is a risk factor.P53 mutation is associated with TNM stage.There were no significant differences in other clinicopathological parameters.There was no difference in the expression of SASH1 and E-cadherin between the negative group and the positive group.The mutation rate of BRAF V600E is up to 80.6%.The TSH level is different between wild type and mutant group.There were no significant differences in other clinicopathological parameters.p53,SASH1 and E-cadherin expression are not related to BRAF V600E mutantConclusion:The expression of SASH1 in papillary thyroid carcinoma cells was lower than that in adjacent normal cells.The expression of SASH1 in multifocal papillary thyroid carcinoma was lower than that in single focal papil lary thyroid carcinoma.SASH1 may be one of the causes of multifocal thyroid papillary carcinomas,and may be used as a molecular marker for the treatme nt of thyroid papillary carcinoma.The average expression of E-cadherin in the tumor cells was lower than that of the paracancerous cells,and the expression was related to the age,but in this experiment the expression of SASH1 was not related to the expression of E-cadherin.E-cadherin and the size of tumor can effect the lymph node metastasis together.The positive rate of p53 expression was not high,and there was no significant difference in the expression of SASH1 and E-cadherin between the positive and negative groups.The mutation rate of the BRAF(V600E)site is high,and the difference between the wild type and the mutant type may represent the degree of normal thyroid function before the operation.SASH1,E-cadherin,p53 and mutations in BRAF may be involved in the development of papillary thyroid carcinoma through different signaling pathways.
