Abstract/Summary:
Objective: To explore the risk factors influencing the clinical aggressive behavior of pituitary adenoma patients,and analyze the correlation between different classification methods and clinical aggressive behavior,so as to lay a foundation for the early judgment and individualized treatment of the clinical aggressive behavior of pituitary adenoma patients.Method: First step: We collected clinical data of 58 cases of primary pituitary adenoma from January 2016 to October 2017,including imaging,serology and pathology examination.Step two:The multimodal classification was established on the basis of imaging and pathological classification of pituitary adenoma.The patients were divided into noninvasive +atypical group,noninvasive +typical group,invasive+atypical group and invasive+ typical group.Step three:After 3 months of comprehensive treatment,the changes of clinical symptoms and signs,thechanges of serum pituitary endocrine hormone levels and the changes of imaging tumor volume 3 months after operation were taken as the basis.The clinical symptom outcome,serological outcome and imaging outcome were evaluated,and the clinical aggressive behaviors of pituitary adenomas in each group were analyzed comprehensively.Step four:Chi-square test and Fisher′s accurate probability method were used for statistics analysis,SPSS software was used to process data,P < 0.05 was regarded as test standard.Result : In this study,6 patients showed aggressive in clinical symptoms outcome and 6 patients showed aggressive in serological outcome.7 patients showed aggressive in imaging.In the multimodal classification of pituitary adenomas,the clinical invasion of pituitary adenomas in invasive + atypical group was significantly higher than that in other groups,differences have statistical significance(P < 0.05).The clinical aggressive behavior of functional pituitary adenoma was significantly different from that of non-functional group,The P value is0.002 in the serological outcome(P < 0.05).The degree of surgical resection was correlated with clinical symptoms and aggressive behavior of imaging,and the differences have statistical significance(P <0.05).Other factors such as single imaging classification,pathological classification,morphological classification,p 53.There was no significantcorrelation between the surgical methods and the clinical aggressive behavior of pituitary adenomas.Conclusion:The multimodal classification method combining with imaging and pathology can indicate the clinical aggressive behavior of pituitary adenoma.This will provide more information for clinicians to develop the best treatment plan,as well as for the future clinical trials and individualized treatment of early intervention therapy.
