Study On The Syndrome Differentiation Of Constitution And Clinical Research Of TCM Gynecology Infertility Patients

ObjectivesTo study the distribution of TCM constitution in infertile patients and analyse the related factors,to assess the feasibility of the constitution distribution in directing clinical treatment through intervening the common distribution of the infertile patients。MethodsFollowwing the method of clinical epidemiological investigation,the research study the distribution of TCM constitution in patients,who were diagnosed with infertility in the Shenzhen Chinese Traditional Medical Hospital and Shenzhen City Maternal and child health care hospital between March 2015 and December 2016,by means of clinical questionnaire,and process the survey data by statistical analysis software to analyse the relationship between the age,duration of disease,etiology,adverse pregnancy history with constitution distribution;using the traditional Chineese medicine to intervene the infertility patients whose TCM constitution have been classified as deficiency of Yang and depression in Qi to assess the feasibility of the constitution distribution in directing clinical treatment.Result(1)The research were collected 340 questionnaires of the infertile patients,including 18 cases with normal constitution(5.3%)and 322 cases with biased constitution(94.7%).And in the 322 patients with biased constitution,there were 79 cases with Yang deficiency(23.2%),70 cases with qi stagnation(20.6%),41 cases with Yin deficiency(12.1%),34 cases with qi deficiency(10%),30 cases with Phlegm dampness(8.8%),25 cases with blood stasis(7.4%),23 cases with damp heat(6.7%),20 cases with unnormal matter(5.9%).(2)In the research,the common syndeome differentiation of TCM in infertile patients from high to low in order is:liver stagnation syndrome(26.5%),kidney yang deficiency syndrome(23.2%),kidney yin deficiency syndrome(19.2%),uterus blood stagnation syndrome(12.0%),Qi and blood deficiency syndrome(9.7%),phlegm dampness syndrome(9.4%).And the research also shows that the syndeome differentiation of TCM has relationship with patients age.patients with infertility aged between 22-30 years old are mainly the type of liver qi stagnation,patients aged between 31-35 years old are mainly the type of liver qi stagnation,kidney yang deficiency,kidney yin deficiency and patients above 35 years old are mainly the type of kidney yang deficiency.(3)There were no statistically significant differences between the age,etiology,duration of disease,adverse pregnancy history with constitution distribution(P>0.05).(4)Study on the correlation between common TCM syndromes and constitution distribution:144 cases of infertile patients are with kidney deficiency syndrome(42.3%)and 198 cases are without kidney deficiency syndrome(57.7%),90 cases of infertile patients are with liver depression syndrome(26.5%)and 250 cases are without liver depression syndrome(72.5%).There are statistically significant differences in the comparision between the distribution of the biased constitution that witn and without kidney deficiency syndrome,P<0.05,The three commonest syndrome in the kidney deficiency group with biased constitution are Yang deficiency(34.2%),qi stagnation(14.7%),yin deficiency(13.6%),There are statistically significant differences In the comparision between the distribution of the biased constitution that witn and without liver depression syndrome,P<0.05,The three commonest syndrome in the liver depression group with biased constitution are Qi stagnation(21%),qi deficiency(16.6%),yin deficiency(13.3%).(5)By intervening the infertile patients with Yang deficiency and Qi stagnation syndrome,we found that it can obviously redress the pregestational biased constitution,improve the endometrial receptivity and increase pregnancy rate after treatment.Conclusion1.Infertile patients are mainly have biased constitution,between them,the Yang deficiency and qi stagnation are the most common constitution.2.The most common clinical TCM syndrome in infertility patients are kidney deficiency and liver depression syndrome.after research,we found that the most common biased constitutions in kidney deficiency patients are Yang deficiency,qi stagnation and yin deficiency syndrome,and the most common biased constitutions in liver depression patients are Qi stagnation,qi deficiency and yin deficiency.3.we can redress the pregestational biased constitution,increase pregnancy rate By intervening the infertile patients with Yang deficiency and Qi stagnation syndrome,and this method is certainly effetive and worth to be popularized widely.
