| Objectives: To investigate the effect of anticoagulation therapy on the pronosis of advanced non-small cell lung cancer with prethrombosis state.Methods:A total of 94 NSCLC patients with abnormal D-Dimer(D-D)were included and randomly divided into two groups.All the patients received a standerd TP(docitaxel combined with cisplatin)regimen while these patients lined in observed arms received anticoagulation additionally according to serum D-Dimer.The first endpoints(chemotherapy response,side effects)and the second endpoints such progression-free survival(PFS)and overall survival(OS)were evaluated respectively.Results:Responsing rate(RR)in observation group was 30.4% and disease control rate(DCR)was 52.2% while they were 29.4% and 50.0% in control group,no significance was observed between the two groups(P >0.05);The incidence of thrombus in observation group and control group was 4.3% and 18.8%,respectively(x2=4.72,P <0.05);The OS was 11.5±4.5 months and 9.2±3.3 monthsin two groups(F=8.176,P =0.005);The PFS was 6.2±4.7 months and 4.4±3.0 months respectively(F=4.777,P=0.031);D-D level was 4.75±2.98mg/ml and 4.94±3.33mg/ml(F=0.088,P =0.767),D-D was negatively correlated with OS(r=-0.521 P<0.001).The major side effect of anticoagulant therapy was the increased risk of bleeding.Conclusion:Chemotherapy combined with anticoagulation can prolong the OS,PFS,reduce the risk of thrombosis.The side effects of anticoagulation therapy can be tolerated,however,the bleeding risk of anticoagulant therapy must be considered. |
