PART ONE:The value of 18F-FDG PET/CT based on Deauville criteria in diagnosis and prognosis of preoperative colorectal cancer with lymph node metastasis Purpose:To study the value of 18F-FDG PET/CT based on Deauville criteria in diagnosis and prognosis of preoperative colorectal cancer with lymph node metastasis Methods:54 patients with colorectal cancer were included in this retrospective analysis.Patients enrolled in the study underwent PET/CT examination without any treatment?eg:surgery, neoadjuvant radiotherapy,chemotherapy,molecule-targeted treatment.?.All of them were approved by pathology within 2 weeks after PET/CT examination. To compare different diagnostic criterias detected by PET/CT,the patients were divided into different groups according to different diagnostic criterias, survival analysis was conducted.Univariate analysis was applied in the relation of prognosis to sex, age, histological type, tumor location,Deauville standard, distant lymph node metastasis,SUVmax.Multivariable logistic regression analysiss were performed in the prediction of risk factors for prognosis. Results:There was no statistical difference between SUVmax:2.06 and Deauville standard in the accuracy of peripheral lymph node metastasis?P>0.05?which were all significant difference compared with SUVmax:2.5?P<0.05?.Univariate analysis showed that prognosis was related to lymph node metastasis and histological type, according to Deauville standard. Conclusion:The diagnostic criterias of 18F-FDG PET/CT by Deauville standard can improve the accuracy of lymph node metastasis in preoperative colorectal cancer.Secondly,it can also be used to assess the prognosis of the patient.PART TWO: The value of postoperative monitoring by 18F-FDG PET/CT and serum CEA on patients with colorectal cancerPurpose: To study the value of postoperative monitoring by 18F-FDG PET/CT and serum CEA on patients with colorectal cancer. Methods: 69 patients with colorectal cancer after surgery were included in this retrospective analysis.Patients enrolled in the study had complete follow-up data about serum CEA and 18F-FDG PET/CT.To compare the number of lesions detected by PET/CT,the patients were divided into two groups according to the normal value of CEA.The patients were divided into different groups according to the cut-off value of CEA obtained by the ROC curve,SUVmax value and lesions sites, survival analysis was conducted. Results: The cut-off value of CEA obtained by the ROC curve was 2.925 ng/ml.Survival analysis based on 2.925 ng/ml showed there was significant statistics differences?p<0.05?.The number of lesion sites including liver metastasis, abdominal implant, abdominal pelvic lymph node metastasis and other metastasis were significant statistics differences between the two groups divided according to the CEA level. However, there was no significant statistics difference?p>0.05?in recurrence and lung metastasis. Conclusion: 18F-FDG PET/CT and serum CEA were useful in recurrence detection and survival prognosis in patients with colorectal cancer.
