Objective:Toinvestigatetheselectionofrecipients,operativetechnique,and perioperative management of lung transplantation for chronic thromboembolicpulmonaryhypertension.Methods:From January2012 to Fbruary2017,8patientswithchronice thromboembolicpulmonaryhypertensionunderwentlungtransplantationat Departmentof Thoracic Srgery,Affiliated Wu uxi People,s Hospital,Nanjing Medical University.Therewere4maleand2 femalepatientsagedfrom 34to64 years.Chestenhancementof CT or CTPA in 8 patients priorto transplantationsuggestsadifferentdegreeofpulmonaryembolism.The meanpulmonaryarterypressure(m PAP)was>30mm Hg,NYHAIIIor I.Six V patients received bilateralsequence lung transplantation(BSLT)under extracorporealmembrane oxygenation(ECMO)support.Two patients receivedsinglelungtransplantation(SLT)),andoneunderextracorporeal membraneoxygenation(ECMO)support.Weanalyzedthedifferencesof pulmonarysystolicpressure,pulmonaryarterydiastolicpressureand mean pulmonaryarterialpressure.Usingpairedttesttocomparetheseindicators,P<0.05 wasconsideredstatisticallysignificant.Results: Thesystolicpressureofpulmonaryarterywasdecreasedfrom (97.25±14.22)mm Hgto(46.00±9.20)mm Hg;thediastolicpressureof pulmonaryarterywasdecreasedfrom(63.50±8.32)mm Hgto(26.50±5.63)mm Hg;Theoxygenationindexincreasedfrom(195.63±31.67)to(408.13±32.51)mm Hgaftertreatment;thedifferencewasstatisticallysignificant(P<0.05).Finally,7patientsweredischargedfrom thehospital,andthe pulmonary valve functioned well.1patientsdiedofshockandsystemicfailure3 days afteroperation.Conclusion:Lungtransplantationiseffectiveinthetreatmentofchronic thromboembolicpulmonaryhypertension,and long-term survivalrequires furtherinvestigations.
