For the traditional intravenous drug configuration,nurses finish that in treatment room,which not only took a lot of time,but also was vulnerable to pollution.To ensure clinical drug safety,Ohio State University Hospital established the world's first parmacy intravenous admixture services.Because of centralized delivery,injection can not be delivered to the clinical in time.With the increasing of time,pH,particles,and main components of injection may change.In this project,we measured the pH,particles,and main components of 25 injections such as vitamin B6,speculated impurity structures,explored the mechanism of incompatibility.We found that the pH value change of the liquid dosage drug injection is smaller than the solid dosage.That was to say,the liquid dosage injection was more stable.The change rate in the pH value of lazazoles injections exceeds 10%,we suggested that lansoprazole,rabeprazole sodium,and pantoprazole sodium injection should be completed within 6 h,6 h,and 8 h respectively.It is presumed that the sulfoxide group in the structure of the lazazoles,which can be easily oxidized to a sulfone group,resulting in an increase in acidity.The change of ?10?m particles is more obvious than the ?25 ?m particles.The increasing of ?10 ?m particles is more obvious.Lansoprazole injection ?10 ?m particles achieve 27 after 3 hours.Rabeprazole Sodium ?10 ?m particles achieve 33 after 1 hours.With the help of LC-MS,it was found that the lazazoles and glucocorticoids degrade significantly with the increase of storage time.After 12 hours,The main components contents of pantoprazole sodium and rabeprazole sodium are both lower than 90%.After 8 hours,the main component content of methylprednisolone sodium succinate is lower than 90%.After being completed,the main component content of dexamethasone sodium phosphate is lower than 90%immediately.We speculate the possible structures of 16 impurities degradations such as AMLZ-1.In summary,after the injection solution is completed,as time goes by,pH,particles,main component content will change.Lazazoles and glucocorticoids change obviously.It has been found that positive and negative ion interactions,pKa and chemical reactions are the three key factors affecting the compatibility of injections.When the compatible drugs are charged with the opposite charge,they tend to attract each other,resulting in polycondensation.When the pKa of two or more drug molecules differ by more than 3.6,it is easier to precipitate.Chemical reactions between some drugs are prone to cause clinical attention.
